目的联合运用行为学测试及静息态功能磁共振成像(rs-f MRI)方法,探讨右侧颞叶癫痫(r TLE)患者警觉相关脑网络特点及其损害。方法对20例r TLE患者和18例对照行注意网络测试(ANT),比较其警觉网络效率。随后行rs-f MRI扫描,运用基于稳定的群组独立成分分析软件(MICA)提取警觉相关脑网络,并行组内分析和组间比较。提取r TLE患者组差异脑区功能连接Z值,与警觉网络效率行相关分析。结果 r TLE患者与对照组ANT测试警觉网络效率差异无统计学意义。r TLE患者组与对照组存在类似的警觉相关脑网络,网络内功能连接减弱的脑区为右顶下小叶、角回,功能连接增强的脑区为右额下回岛盖部、中央沟盖、额中回、背外侧额上回、楔叶、枕上回等脑区。右楔叶功能连接Z值与警觉网络效率相关。结论 r TLE患者存在警觉相关脑网络损害,可通过增强其他脑区功能连接进行代偿。
Objective To explore the characteristics and damage of alertness-related brain networks in patients with right temporal lobe epilepsy (r TLE) using behavioral tests and resting-state functional magnetic resonance imaging (rs-f MRI). Methods Twenty patients with T-TLE and 18 controls were included in the ANT. The alertness network efficiency was compared. Followed by a rs-f MRI scan, the alert-dependent brain network was extracted using a stable group independent component analysis software (MICA), in parallel with in-group analysis and between-group comparisons. Extract the Z value of functional brain connectivity in r TLE patients group, and correlate with the alertness network efficiency. Results There was no significant difference in alertness network efficiency between ANT and r TLE patients. r TLE patient group and the control group there is a similar alert-related brain network, network connectivity is weakening of the brain area of the right parietal lobule, angle gyrus, functional connectivity enhancement of the brain area for the right frontal lower back cover, central ditch cover , The amount of back, back forehead on the back, wedge leaves, pillow back and other brain areas. The right wedge leaf function connection Z value is related to the alertness network efficiency. Conclusion There is vigilance-related brain network impairment in patients with r TLE, which may be compensated by enhancing functional connectivity in other brain regions.
