目的:探讨慢性心力衰竭患者门诊治疗的依从性。方法:通过前瞻性登记表调查方式,对患者进行为期1~8年,间隔2~8周的门诊随访治疗,探讨对患者依从性的因素影响,及对心力衰竭控制和预后的影响。结果:812例患者中,736例(90.1%)完成随访。分析发现职业与治疗依从性无关,卡方检验无统计学差异(P>0.05);执行医嘱的依从性好的患者,其心力衰竭的控制情况显著优于依从性差的患者(P<0.05),而且再住院率和死亡率显著低于依从性差的患者(P<0.05);药物相关因素成为影响患者门诊随访治疗依从性的最重要因素。结论:慢性心力衰竭患者的门诊治疗的依从性,主要决定于用药医嘱的繁杂程度,依从性差,心力衰竭的控制差,预后也差。
Objective: To investigate the compliance of outpatient treatment in patients with chronic heart failure. Methods: According to the prospective registration form investigation, patients were followed up for 1 to 8 years at intervals of 2 to 8 weeks to explore the influence on the patients’ compliance and on the control and prognosis of heart failure. Results: Of the 812 patients, 736 (90.1%) were followed up. The results showed that there was no significant difference between occupation and treatment compliance (P> 0.05). The patients with good adherence to doctor’s advice had significantly better control over heart failure than those with poor compliance (P <0.05) Moreover, the rate of rehospitalization and mortality was significantly lower than that of patients with poor compliance (P <0.05). The drug-related factors were the most important factors influencing the follow-up treatment compliance in patients. Conclusion: The compliance of outpatient treatment in patients with chronic heart failure mainly depends on the complexity of medication orders, poor compliance, poor control of heart failure and poor prognosis.
