目的:测算不同程度药品不良反应(ADR)造成的相关费用并进行比较分析。方法:采用回顾性方式,将收集的ADR病例统计筛选后分为A组(严重组)、B组(治愈组)及C组(自愈组),利用项目法和人力资本法分别测算三组病例由ADR造成的直接和间接费用,忽略隐性成本。结果:A组病例医保支付比例为30.94%,导致住院治疗时间平均11.3d,平均每例需要支付直接费用6,923.78元;B组病例医保支付比例为43.17%,平均每例需要支付直接费用83.75元;C组病例医保支付比例为20.00%,平均每例需要支付直接费用38.81元。敏感性分析提示,药品降价有助于减轻由ADR造成的直接经济损失。结论:ADR造成的费用(特别是严重病例)不容忽视,开展经济学研究应该是ADR监测工作的重点之一。
OBJECTIVE: To estimate the costs caused by different degrees of adverse drug reactions (ADRs) and to conduct comparative analysis. Methods: A retrospective analysis of the collected ADR cases was screened and divided into A group (severe group), B group (Cure group) and C group (self-healing group). The project method and human capital method were used to calculate three groups Case Direct and indirect costs caused by ADR, ignoring hidden costs. Results: The proportion of medical insurance in group A was 30.94%, resulting in an average of 11.3 days in hospitalization, with an average cost of 6,923.78 yuan for each case. The proportion of medical insurance in group B was 43.17%, with an average of 83.75 yuan for each case. C group medical insurance payment ratio of 20.00%, on average, each case need to pay a direct cost of 38.81 yuan. Sensitivity analysis suggests that drug price cuts help to mitigate the direct economic loss caused by ADRs. Conclusion: The costs caused by ADR (especially serious cases) should not be neglected. To carry out economic studies should be one of the focuses of ADR monitoring.
