目的 :探讨一般人群血清维生素D和甲状旁腺激素与心率收缩压乘积的关系。方法 :在前期横断面研究的基础上,抽取苏州市621名年龄≥40岁常住居民为研究对象。采用方差分析和广义线性模型分析不同25-羟基维生素D[25(OH)D]、甲状旁腺激素(PTH)水平分组下的心率、收缩压及心率收缩压乘积(RPP)的关系。结果:25(OH)D<10μg/L组与≥20μg/L组相比,心率增快(5.0±1.2)次/min(P<0.01),RPP增加(754.3±187.8)次/min·mm Hg(P<0.01)。当PTH处于高水平时,25(OH)D<10μg/L组与≥20μg/L组相比,心率增快10.7次/min(P=0.001),RPP增加1 279.4次/min·mm Hg(P=0.011);调整多个因素后,心率仍增快8.2次/min(P=0.017),RPP增加1 041.8次/min·mm Hg(P=0.06)。当PTH处于低水平时,25(OH)D<10μg/L组与≥20μg/L组相比,心率增快3.7次/min(P=0.018),RPP增加639.5次/min·mm Hg(P=0.007);调整多因素后RPP仍增加595.7次/min·mm Hg(P=0.012)。结论 :维生素D缺乏者心率加快、RPP增加、心脏做功增加,尤其维生素D缺乏与高水平的PTH同时存在时对以上效应可能起到协同作用。
Objective: To investigate the relationship between serum vitamin D, parathyroid hormone and heart rate systolic pressure in general population. Methods: Based on the previous cross-sectional studies, 621 permanent residents aged 40 years and older were selected as the research subjects. ANOVA and generalized linear models were used to analyze the relationship between heart rate, systolic pressure, and heart rate systolic pressure (RPP) under different 25-hydroxyvitamin D [25 (OH) D] and parathyroid hormone (PTH) groups. RESULTS: Compared with ≥20μg / L group, the heart rate increased (5.0 ± 1.2) times / min (P <0.01) and the RPP increased (754.3 ± 187.8) times / min · mm in 25 (OH) D group Hg (P <0.01). When PTH was high, heart rate increased 10.7 times / min (P = 0.001) in 25 (OH) D <10μg / L group and ≥20μg / L group, and RPP increased 1 279.4 times / min · mm Hg P = 0.011). After adjusting for a number of factors, the heart rate still increased 8.2 times / min (P = 0.017) and the RPP increased 1 041.8 times / min mm Hg (P = 0.06). After adjusting for multiple factors, RPP still increased by 595.7 times / min · mm Hg (P = 0.012). Conclusion: The heart rate of vitamin D deficiency is increased, RPP is increased, heart work is increased, especially vitamin D deficiency and high levels of PTH co-exist when the above effects may play a synergistic effect.
