下肢动脉硬化闭塞症的多层螺旋(HD 750)CTA和3.0T磁共振3D CE-MRA的对比研究

目的对比研究多排螺旋增强血管造影CTA和自动移床三维动态增强磁共振血管造影3D CE-MRA对诊断下肢动脉硬化闭塞症的应用价值。方法收集2012年5月-2014年11月共38例下肢动脉硬化闭塞症患者,每例患者在不超过2周期间内都进行了HD750 CTA和3.0T 3D CE-MRA检查,其中15例同期进行了DSA检查。回复性分析相关患者影像数据,并对每例双侧下肢动脉划分为3个区域、23个节段的CTA和CE-MRA进行多种后重建图像进行比较。结果全部病例的CTA和CE-MRA均可以准确评估下肢动脉硬化闭塞部位及程度,共显示轻度狭窄8例,中度狭窄14例,重度狭窄12例和4例动脉闭塞,符合临床诊断要求。MRA对狭窄程度评估更准确,CTA对于附壁斑块的显示及斑块成分的评估有优势。结论 CTA和3D CE-MRA对诊断下肢动脉硬化闭塞症的的评估都是可靠和准确的,二者各有优势,互补使用可为临床提供更多的诊断信息,能够为临床采取及时有效的治疗提供更准确可靠的影像依据。 Objective To compare the clinical value of multi-slice spiral enhanced angiography (CTA) and three-dimensional dynamic enhanced magnetic resonance angiography (3D CE-MRA) in diagnosis of arteriosclerosis obliterans in lower extremities. Methods A total of 38 patients with arteriosclerotic occlusive disease of the lower extremities were collected from May 2012 to November 2014. Each patient underwent HD750 CTA and 3.0T 3D CE-MRA examination within a period of no more than 2 weeks, of which 15 were performed concurrently DSA check. Retrospective analysis of patient-related imaging data, and each case of bilateral lower extremity artery is divided into three regions, 23 segments of CTA and CE-MRA after a variety of post-reconstruction images were compared. Results CTA and CE-MRA in all cases were able to accurately assess the location and extent of lower extremity arteriosclerosis occlusion, showing a total of 8 cases of mild stenosis, moderate stenosis in 14 cases, severe stenosis in 12 cases and 4 cases of arterial occlusion, in line with clinical diagnosis requirements. MRA is more accurate assessment of the degree of stenosis, CTA for the display of plaque and plaque composition assessment has an advantage. Conclusions Both CTA and 3D CE-MRA are reliable and accurate for the diagnosis of arteriosclerosis obliteration in lower extremities. Both of them have advantages and complement each other, which can provide more information for clinical diagnosis and provide timely and effective treatment for clinical treatment Provide a more accurate and reliable image basis.
