Low -grade endometrial stromal sarc oma (LGESS)and endometriosis are two conditions of different prognostic significance that are usually not di fficult to distinguish histologically.We present the case of a 38-year -old woman who underwent laparotomy afte r a diagnosis of en-dometrial stromal neoplasm on uterine curettings.Uterine enlargement and regional and paraao rtic lymphadenopathy were found.Pathology showed adenom yotic -like myome-trial hypertrophy and menstrual -li ke shedding of tumor in involved lymph nodes.The finding of large areas of en-dometrial stromal cells without accompanying glands form-ing tumorous masses in both sites supported the diagnosis of LGESS.We believe that this is the fir st time that these phenomena have been reported in LGESS.In these cases,adenomyotic myometrial hypertrophy and menstrual shed-ding have been used to distinguish en dometriosis from LGESS,but do not appear to be absolut e criteria.En-dometriosis and LGESS are undoubted ly separate entities,but occasionally may resemble each o ther.
