本文报道10例胎儿宫内生长迟缓(IUGR)胎盘和20例正常胎盘的组织计量,并对其中经剖宫产的4例IUGR产妇取胎盘床活检,进行螺旋动脉形态学观察。组织计量结果显示IUGR组胎盘重量、胎盘容积、绒毛表面积及绒毛毛细血管表面积均较对照组显著减小。但IUGR胎盘绒毛发育未显示成熟障碍,且两组间胎儿单位体重的胎盘重量、绒毛表面积和绒毛毛细血管表面积无显著差异。10例IUGR胎盘中慢性绒毛炎3例;4例胎盘床活检中,除1例取材不当外,余3例均有程度不等的螺旋动脉病理性改变。提示母体血管病变和原因不明的绒毛炎可能导致胎儿IUGR的发生。
This article reports 10 cases of intrauterine growth retardation (IUGR) placenta and 20 cases of normal placenta tissue measurement, and cesarean section in which 4 cases of IUGR maternal take placental biopsy for morphological observation of spiral arteries. Tissue measurement results showed that placental weight, placental volume, villus surface area and villus capillary surface area in IUGR group were significantly decreased compared with those in control group. However, development of placental villi of IUGR did not show a maturational disorder, and there was no significant difference in placental weight, villus surface area and villus capillary surface area between the two groups. 3 cases of chronic villus inflammation in 10 cases of IUGR placenta; 4 cases of placental bed biopsy, except 1 case of inappropriate material, the other 3 cases have varying degrees of pathological changes of the spiral arteries. Prompted maternal vascular disease and unexplained villus inflammation may lead to the occurrence of fetal IUGR.
