目的探讨PET/CT对尘肺病的诊断价值。方法分析1例经PET/CT误诊为肺癌的尘肺病患者的临床资料、影像学、实验室和手术病理结果,并复习国内外相关文献。结果患者为60岁男性,因体检发现双肺多发结节,行PET/CT提示右肺小结节及纵隔、双肺门小淋巴结,糖代谢增高,考虑肺癌。入院后行胸腔镜下右肺上叶楔形切除术,术后病理证实为尘肺病。结论尘肺病PET/CT发现的高代谢病灶容易误诊为肺癌,其对尘肺病的诊断价值尚不确切,需要更深入的研究,探索新的解决方案。
Objective To investigate the diagnostic value of PET / CT for pneumoconiosis. Methods One patient with pneumoconiosis who had been misdiagnosed as lung cancer by PET / CT was reviewed. The clinical data, imaging, laboratory and surgical pathology were reviewed. Relevant literatures at home and abroad were reviewed. Results The patients were 60-year-old men. Multiple lung nodules were found due to physical examination. PET / CT showed small nodules of the right lung and mediastinum and small hilar lymph nodes of the hilar. The glucose metabolism was increased and lung cancer was considered. After admission, the right thoracoscopic wedge resection of the right upper lobe, postoperative pathology confirmed as pneumoconiosis. Conclusions The high metabolic lesions found in PET / CT of pneumoconiosis are easily misdiagnosed as lung cancer. The diagnostic value of PET / CT for pneumoconiosis is not clear. More research is needed to explore new solutions.
