目的比较CT血管造影(CAT)和经颅多普勒超声(TCD)诊断颅内血管狭窄的准确性。方法选取200例可疑颅内血管狭窄的患者,发病3d内分别进行CTA和TCD检查,比较2种方法诊断颅内血管狭窄的准确性。结果 CAT诊断颅内血管狭窄的灵敏度为68.4%,特异度为65.4%,准确度为68.0%,TCD诊断颅内血管狭窄的灵敏度为52.9%,特异度为57.7%,准确度为53.5%,CAT诊断的灵敏度及准确度均高于TCD,差异均有统计学意义(P<0.05),CAT诊断的特异度高于TCD,但差异无统计学意义(P>0.05)。结论 CAT在诊断颅内血管狭窄中相对TCD具有较高的灵敏度和准确度,具有较高的诊断价值,在临床中值得推广应用。
Objective To compare the accuracy of CT angiography (CAT) and transcranial Doppler ultrasound (TCD) in the diagnosis of intracranial vascular stenosis. Methods Totally 200 patients with suspected intracranial stenosis were selected. CTA and TCD were performed respectively within 3 days of onset. The accuracy of 2 methods in diagnosing intracranial vascular stenosis was compared. Results The sensitivity, specificity and accuracy of TCD in diagnosing intracranial vascular stenosis were 68.4%, 65.4% and 68.0%, respectively. The sensitivity and specificity of TCD in diagnosing intracranial vascular stenosis were 52.9%, 57.7% and 53.5% The diagnostic sensitivity and accuracy were higher than TCD, the difference was statistically significant (P <0.05), CAT diagnostic specificity was higher than TCD, but the difference was not statistically significant (P> 0.05). Conclusion CAT in the diagnosis of intracranial vascular stenosis relative TCD has a higher sensitivity and accuracy, has a high diagnostic value, it is worth to promote the use of the clinic.
