目的运用PDCA循环管理,减少呼吸机相关性肺炎的发生。方法将重症监护病房2007年应用呼吸机患者设为对照组,2008年应用呼吸机患者设为试验组,对试验组应用PDCA循环管理模式管理。结果呼吸机相关性肺炎感染率、死亡率由2007年的46.0%、32.0%,降至为2008年19.5%、21.1%,差异有统计学意义(2χ=52.74,P<0.01);应用呼吸机患者的平均治疗费用2007年为45 728元,2008年为38 707元,下降率为15.4%。结论通过PDCA循环管理,使应用呼吸机患者呼吸机相关性肺炎感染率大大降低,是一种科学、有效的管理方法 。
Objective To use PDCA cycle management to reduce the occurrence of ventilator-associated pneumonia. Methods The patients with ventilator in ICU in 2007 were set as the control group. In 2008, the patients with ventilator were set as the experimental group, and the experimental group were administered PDCA cycle management model. Results The infection rate and death rate of ventilator-associated pneumonia dropped from 46.0% and 32.0% in 2007 to 19.5% and 21.1% in 2008, respectively, with significant difference (2χ = 52.74, P <0.01) The average cost of treatment of patients was 45 728 yuan in 2007, 38 707 yuan in 2008, the rate of decline was 15.4%. Conclusions PDCA cycle management can reduce the rate of ventilator-associated pneumonia in ventilator patients. It is a scientific and effective management method.
