目的 :探讨原发性高血压 (EH)早期肾损害的影响因素。方法 :以 10 0例EH患者和 5 0例正常人作为研究对象 ,进行肾功能检测、2 4h动态血压监测 (ABPM )和超声多普勒肾血流检查。以尿白蛋白排泄率(UAER)作为因变量 ,性别、年龄、病程、体重指数 (BMI)、血糖 (BS)、肾血流参数、ABPM参数作为自变量 ,用逐步回归方法进行多元回归分析。结果 :多元逐步回归分析表明 ,2 4h收缩压 (SBP)均值、BS、BMI、肾叶间动脉阻力指数 (RI)对EH早期肾损害有明显影响 (R2 分别为 0 .4 2 5、0 .4 5 4、0 .5 2 2和 0 .5 4 1,均P <0 .0 1)。结论 :EH早期肾损害的影响因素是 2 4hSBP均值、BS、BMI、RI。
Objective: To investigate the influencing factors of early renal damage in essential hypertension (EH). Methods: A total of 100 EH patients and 50 normal controls were enrolled in this study. Renal function tests, 24-h ambulatory blood pressure monitoring (ABPM) and ultrasound Doppler renal blood flow were performed. Urea excretion rate (UAER) as the dependent variable, gender, age, course of disease, body mass index (BMI), blood glucose (BS), renal blood flow parameters, ABPM parameters as independent variables, using stepwise regression analysis of multiple regression analysis. Results: Multivariate stepwise regression analysis showed that mean systolic blood pressure (SBP), BS, BMI and RI were significantly affected at early stage of EH (R2 = 0.42 5,0. 4 5 4, 0 .5 2 2 and 0 .5 4 1, both P <0. Conclusion: The influencing factors of early renal damage in EH are 24 hSBP mean, BS, BMI and RI.
