目的研究SWOT分析方法在社区卫生服务中心与上级综合医院间的胃肠肿瘤“双向转诊”中的意义。方法选择胃癌及大肠癌患者274例及华漕社区卫生服务中心的33名社区医生作为研究对象。运用SWOT分析法,分析华漕社区卫生服务中心与闵行区医院建立双向转诊制度的优势、劣势、机会及威胁,探索社区卫生服务中心与区域医学中心建立规范的胃肠肿瘤“双向转诊”机制的可行性。结果实施双向转诊后转之前到社区就诊患者增加了88.95%。自行就诊所花费的平均费用为241.33元,转诊的为150.08元,比较差异有统计学意义(P<0.05)。对社区医院提供的医疗服务的满意度为100%,对上级医院诊疗过程的满意度为98.5%,对社区医学生医疗水平的满意度为70.1%,对转诊作用的满意度为92.3%。结论肿瘤早发现双向转诊机制的建立,不仅能够提高社区居民胃癌和大肠癌的早发现率,有效控制医疗费用上涨,还可以改善专科医院和社区卫生服务站的服务质量,同时对我国的社区卫生服务的健康发展、规范卫生服务操作程序、提高服务质量起着积极的引导和推动作用。
Objective To study the significance of SWOT analysis in gastrointestinal cancer “two-way referral” between community health service centers and superior general hospitals. Methods A total of 274 patients with gastric cancer and colorectal cancer and 33 community doctors from Huacao community health service center were selected as research objects. Using SWOT analysis, this paper analyzes the advantages, disadvantages, opportunities and threats of establishing a two-way referral system between Huacao Community Health Service Center and Minhang District Hospital, and explores the establishment of a standardized gastrointestinal tumor in the community health service center and the regional medical center "Mechanism of feasibility. Results After the two-way referral to the community visits before the transfer of patients increased by 88.95%. The average cost of self-clinics was 241.33 yuan, referring to 150.08 yuan, the difference was statistically significant (P <0.05). Satisfaction with the medical service provided by the community hospital was 100%, satisfaction with the clinic process at the superior hospital was 98.5%, satisfaction with the medical level of the community medical student was 70.1% and satisfaction with the referral was 92.3%. Conclusion The establishment of a two-way referral mechanism for early detection of tumor can not only improve the early detection rate of gastric cancer and colorectal cancer among residents in the community, but also effectively control the rising medical costs. It can also improve the service quality of specialist hospitals and community health service stations, The healthy development of health services, standardize the operation procedures of health services, improve service quality plays a positive role in guiding and promoting.
