目的 研究嗜酸粒细胞性支气管炎 (EB)气道管壁结构改变与转化生长因子 β1(TGF β1)和血小板衍化生长因子 (PDGF)在气道黏膜中表达之间的关系。方法 选择 12例EB(A组 )、10例哮喘 (B组 )和 10例早期周围型肺癌 (C组 )患者 ,经纤维支气管镜作肺下叶段支气管黏膜活组织检查 (简称活检 ) ,C组患者活检取健侧肺。苏木精 伊红 (HE)染色观察气道上皮下基膜 (SBM )厚度 ;免疫组化技术检测TGF β1和PDGF在气道黏膜中的表达。结果 A组SBM厚度为 (6 3± 1 4 ) μm、B组为 (8 2± 1 5 ) μm、C组为 (4 1± 1 2 ) μm ,三组比较差异有显著性 (P <0 0 5 ) ;A组气道黏膜以及黏膜下层中TGF β1和PDGF每平方毫米阳性细胞数分别为 (5 9± 9)个、(4 7± 7)个、B组分别为(85± 12 )个、(76± 11)个、C组分别为 (31± 4 )个、(2 0± 3)个 ,三组比较差异有显著性 (P <0 0 5 ) ,且TGF β1和PDGF与SBM厚度呈正相关关系 (r分别 =0 76、0 5 2 ,P <0 0 5 )。结论 EB患者可以出现类似于支气管哮喘的气道SBM增厚的病理改变 ,且TGF β1和PDGF在EB气道黏膜下纤维化的病理改变中发挥了重要作用。
Objective To study the relationship between the changes of airway wall structure and the expressions of transforming growth factor β1 (TGFβ1) and platelet-derived growth factor (PDGF) in airway mucosa of eosinophilic bronchitis (EB). Methods Twelve cases of EB (group A), 10 cases of asthma (group B) and 10 cases of early stage peripheral lung cancer (group C) were selected for bronchial mucosa biopsy (referred to as biopsy) Group patients biopsy take healthy lungs. The thickness of airway epithelial basement membrane (SBM) was observed by hematoxylin and eosin (HE) staining. The expression of TGF-β1 and PDGF in airway mucosa was detected by immunohistochemistry. Results The thickness of SBM in group A was (6 3 ± 1 4) μm, in group B was (82 ± 1 5) μm, in group C was (4 1 ± 1 2) μm, the difference was significant (5 9 ± 9), (47 ± 7), respectively, in group A and (85 ± 12) in group B, respectively (31 ± 4) and (20 ± 3) in group C, respectively (P <0.05), and there was a significant difference between the three groups (P <0.05) Thickness showed a positive correlation (r = 0 76,0 5 2, P <0 05). Conclusion The pathological changes of airway SBM thickening similar to bronchial asthma may occur in EB patients, and TGF β1 and PDGF play an important role in the pathological changes of EB airway submucosal fibrosis.
