为探讨类风湿关节炎(RA)患者外周血和滑液中可溶性及膜型CD137的表达变化及其与RA患者临床指标的相关性。采用酶联免疫吸附试验(ELISA)法及三色荧光染色流式细胞仪(FCM)检测了32例RA患者外周血、34例健康人外周血、10例RA患者滑液和8例OA患者滑液中可溶性CD137及CD3+CD4+CD137+T细胞的表达水平,并分析与RA的各项临床指标的相关性。与健康对照组相比,CD3+CD4+CD137+T细胞及sCD137在RA患者活动期和缓解期的外周血和滑液中均明显升高(P<0.05);而外周血中缓解期的CD3+CD4+CD137+T细胞水平及sCD137水平与活动期相比则呈反向变化。RA外周血中sCD137水平与CD3+CD4+CD137+T细胞水平无相关性。RA滑液中的sCD137及CD3+CD4+CD137+T细胞均与RA与活动性指标ESR、炎症指标CRP及疾病活动指数Stroke指数均呈正相关(P<0.05)。外周血中的仅有sCD137与RA慢性期指标RF呈正相关关系(P<0.05)。可见RA患者关节的炎症反应可能与CD137在滑液中的免疫表达有关。
To investigate the changes of soluble and membranous CD137 expression in peripheral blood and synovial fluid of patients with rheumatoid arthritis (RA) and its correlation with the clinical parameters of RA patients. Peripheral blood samples from 32 patients with RA, peripheral blood from 34 healthy subjects, synovial fluid from 10 patients with RA, and 8 patients with OA were detected by enzyme-linked immunosorbent assay (ELISA) and trichrome fluorescent staining (FCM) Soluble CD137 and CD3 + CD4 + CD137 + T cells in human serum were detected by flow cytometry and their correlations with clinical parameters of RA were analyzed. Compared with healthy controls, CD3 + CD4 + CD137 + T cells and sCD137 were significantly increased in both peripheral blood and synovial fluid of active and remission RA patients (P <0.05), while CD3 + CD4 + CD137 + T cells and sCD137 levels compared with the active phase was the reverse change. The level of sCD137 in RA peripheral blood was not correlated with the level of CD3 + CD4 + CD137 + T cells. RA synovial fluid of both sCD137 and CD3 + CD4 + CD137 + T cells were positively correlated with RA, ESR, CRP and Stroke index (P <0.05). There was a positive correlation between sCD137 in peripheral blood and RF in chronic stage of RA (P <0.05). It can be seen that the inflammatory response of RA patients may be related to the immune expression of CD137 in synovial fluid.
