目的 采用GDxVCC系统检测正常人、可疑开角型青光眼(suspectedopenangleglaucoma ,SOAG)以及原发性开角型青光眼(primaryopenangleglaucoma,POAG)患者的视盘周围视网膜神经纤维层(retinalnervefiberlayer,RNFL)厚度,并进行对比分析,探讨GDxVCC系统在青光眼早期诊断中的价值。设计 前瞻性对照研究。研究对象 4 2例SOAG ,36例POAG以及年龄相匹配的36例正常人参加此项研究。方法 所有参加者进行视野与GDxVCC系统检查,选择一眼进行分析。采用方差分析,对SOAG、POAG、正常人的RNFL参数进行两两比较。主要指标 GDxVCC检测RNFL厚度。结果 POAG组与正常组间所有RNFL参数差异均有极显著意义(P =0 0 0 0 ) ;POAG组与SOAG组间所有RNFL参数差异均有极显著意义(P=0 0 0 0 ) ;SOAG组与正常组间椭圆平均值差异无统计学意义(P =0 2 34) ,但其上方平均值与下方平均值及神经纤维指数差异均有统计学意义(P =0 0 4 4、P =0 0 4 5、P =0 0 35 ) ,TSNIT标准差有极显著意义(P =0 0 0 0 )。结论 GDxVCC系统能更早地发现青光眼性RNFL结构损害,不仅可用于青光眼诊断和随访,亦可用于对可疑青光眼进行追踪。
Objective To detect the thickness of retinal nerve fiber layer (RNFL) around the optic disc in patients with suspected openopenangular glaucoma (SOAG) and primaryopenangleglaucoma (POAG) using the GDxVCC system. To investigate the value of GDxVCC system in the early diagnosis of glaucoma. Design prospective controlled study. Forty-two SOAG patients, 36 POAG patients, and 36 healthy matched age-matched normal subjects were enrolled in the study. Methods All participants underwent visual field and GDxVCC system exams and chose one for analysis. ANOVA was used to compare the RNFL parameters of SOAG, POAG and normal subjects. The main indicator GDxVCC detection RNFL thickness. Results All RNFL parameters were significantly different between POAG group and normal group (P = 0 0 0 0). All RNFL parameters between POAG group and SOAG group were significantly different (P = 0 0 0 0). SOAG There was no significant difference between the mean values of the ovals in the group and the normal group (P = 0 2 34), but there was a statistically significant difference between the average of the above and the average of the lower part and the index of the nerve fiber (P = 0 0 4 4, P = 0 0 4 5, P = 0 0 35). The standard deviation of TSNIT was significant (P = 0 0 0 0). Conclusion The GDxVCC system can detect structural damage of glaucomatous RNFL earlier and can be used not only for diagnosis and follow-up of glaucoma but also for tracking of suspected glaucoma.
