目的探讨新疆农村居民疾病经济负担现状及住院费用的影响因素。方法采用分层抽样的方法,分别抽取南北疆共1258户农村居民,对住院人员的住院费用情况进行调查并进行统计学分析。结果共调查5 329人,有365人在2014年因伤病住院,住院率为7.79%;单因素分析显示,婚姻状况、文化程度、医保类型、住院类别、月人均收入水平与住院费用存在相关性(P<0.05)。住院费用多因素logistic回归分析显示,住院费用受婚姻状况、医保类型、住院类别、月人均收入和是否报销医疗费用的影响。结论新疆农村医保制度切实解决了低收入弱势人群的参合问题;政府在加大基层医疗机构的财政投入时,应合理控制高额病例的住院费用;应积极探索多途径补偿方式。
Objective To explore the status quo of economic burden of rural residents in Xinjiang and the influencing factors of hospitalization costs. Methods By stratified sampling method, a total of 1258 rural residents were selected from the northern and southern Xinjiang respectively. The hospitalization expenses of inpatients were investigated and statistically analyzed. Results A total of 5 329 people were surveyed, of whom 365 were hospitalized for injuries and were hospitalized in 2014 with a hospitalization rate of 7.79%. Univariate analysis showed that marital status, educational level, type of health care insurance, hospitalization category and monthly per capita income were related to hospitalization expenses (P <0.05). Multivariate logistic regression analysis of hospitalization costs showed that hospitalization expenses were affected by marital status, type of medical insurance, hospitalization category, per capita monthly income, and whether medical expenses were reimbursed. Conclusion Xinjiang’s rural health insurance system effectively solves the problem of participation of low-income and disadvantaged groups. The government should rationally control the hospitalization expenses of high-income cases when it increases the financial input of primary health care institutions. It should actively explore ways of compensation through multiple channels.
