目的探讨褪黑素对糖尿病患者氧化应激的抑制作用及糖脂代谢的影响。方法1998-12~2003-04东南大学附属中大医院采用随机、单盲、安慰剂平行对照的方法,观察褪黑素及安慰剂对91例糖尿病患者血清丙二醛(MDA)及超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSHPx)的影响,并分析上述影响与糖脂代谢变化之间的关系。结果治疗前糖尿病患者血清SOD、GSHPx水平下降,MDA水平上升。褪黑素组8周处理后血清SOD和GSHPx水平上升,而MDA水平明显下降,与处理前相比差异有显著性;安慰剂组处理前后患者血清SOD、GSHPx及MDA水平无明显变化。褪黑素组空腹血糖和HbA1c水平较治疗前下降,而餐后2h血糖、TG、TC、HDLC水平与治疗前相比无明显变化,LDLC虽有下降趋势,但与治疗前相比无统计学意义。安慰剂组处理前后患者血糖及血脂谱无明显变化。结论褪黑素对糖尿病患者氧化应激有明显的抑制作用,且有益于糖代谢的稳定。
Objective To investigate the inhibitory effect of melatonin on oxidative stress in diabetic patients and the effects of glucose and lipid metabolism. Methods A total of 91 diabetes mellitus patients with melatonin and placebo were randomly divided into control group, (SOD), glutathione peroxidase (GSHPx), and analyze the relationship between the above effects and changes in glucose and lipid metabolism. Results Before treatment, serum SOD, GSHPx levels decreased and MDA level increased in diabetic patients. Serum levels of SOD and GSHPx were elevated in melatonin group after 8 weeks of treatment, while the level of MDA was significantly decreased compared with that before treatment. Serum levels of SOD, GSHPx and MDA did not change significantly in the placebo group before and after treatment. The levels of fasting blood glucose and HbA1c in melatonin group were lower than those before treatment, while the levels of blood glucose, TG, TC and HDLC in post-prandial 2h group had no significant difference compared with those before treatment, although LDLC had a decreasing trend, but no statistical difference significance. There was no significant change in blood glucose and lipid profile of patients in the placebo group before and after treatment. Conclusion Melatonin has obvious inhibitory effect on oxidative stress in diabetic patients and is beneficial to the stability of glucose metabolism.
