目的 :探讨肝损害型毒蕈中毒患者的肝脏功能的动态变化。方法 :对 1 2起肝损害型毒蕈中毒事件中 38个住院患者动态观察肝肾功能 ,凝血酶原时间 ,心肌酶学等指标的变化 ,并与其他类型毒蕈中毒进行比较。结果 :在中毒后 3d内 ,肝损害型毒蕈中毒患者的谷丙转氨酶 (ALT)和谷草转氨酶 (AST)均显著升高 ,与其他类型毒蕈中毒有显著差异 ;食用量大小与肝功能损害程度有关 ;肝损害型毒蕈中毒死亡率高 ,TBA≥ 50 μmol/L ,PT≥ 40s,AST≥ 80 0u/L且AST/ALT≥ 0 .75是预后不良的重要指标。结论 :动态观察肝损害型毒蕈中毒患者肝脏功能指标 ,对诊断及预后判定有重要指导意义。
Objective: To investigate the dynamic changes of liver function in patients with liver damage poisoned mushroom poisoning. Methods: Thirty-eight inpatients with hepatotoxicity were divided into two groups. The changes of liver and kidney function, prothrombin time, myocardial enzyme and other indexes were observed dynamically and compared with other types of poisoned mushroom poisoning. Results: ALT and AST were significantly increased in patients with liver damage poisoning poisoning within 3d days after poisoning, which was significantly different from other types of poisoning mushroom poisoning. The amount of food consumption and liver function impairment The degree of mortality was higher in patients with liver damage and poisoning, with a TBA≥50 μmol / L, PT≥40s, AST≥80 0u / L and AST / ALT≥0.75, which were the important indicators of poor prognosis. Conclusion: The dynamic observation of hepatic impairment in patients with liver damage poisoning has important guiding significance for the diagnosis and prognosis.
