目的:观察氟比洛芬酯与芬太尼超前镇痛对于无痛人流术患者的镇痛效果并进行对比。方法:拟行无痛人流术患者90例,年龄18~35岁,ASAⅠ~Ⅱ级,随机分为氟比洛芬酯组(A组)、芬太尼组(B组)及对照组(C组),每组30例。A组麻醉诱导前10 min静注氟比洛芬酯50 mg,B组麻醉诱导前10 min静注芬太尼1μg/kg,C组麻醉诱导前不给予药物。分别记录诱导前(T0)、手术开始时(T1)、扩宫口时(T2)、宫腔负压吸引时(T3)、手术结束时(T4)、唤醒时(T5)各时间点的平均动脉压(MAP)、心率(HR)、脉搏血氧饱和度(SpO2)。记录丙泊酚总用量、诱导时间、唤醒时间、术中患者体动和呼吸抑制的发生情况。记录术后患者VAS评分。结果:A组术中MAP、HR、SpO2情况优于B、C两组;A、B两组丙泊酚总用量、VAS评分、术中体动次数均低于C组,且A组唤醒时间低于B、C两组;B组呼吸抑制发生次数显著高于A、C两组。结论:氟比洛芬酯与芬太尼超前镇痛用于无痛人流术均可得到较好的镇痛效果,但使用氟比洛芬酯无芬太尼的呼吸抑制作用,更适合无痛人流术。
Objective: To observe and compare the analgesic effect of flurbiprofen axetil and fentanyl advanced analgesia in patients with painless abortion. Methods: 90 patients with painless abortion were enrolled in this study. They were randomly divided into four groups: the flurbiprofen axetil group (group A), the fentanyl group (group B) and the control group (group C) Group), 30 cases in each group. Group A was treated with flurbiprofen axetil 50 mg 10 minutes before induction of anesthesia, and fentanyl 1 μg / kg 10 minutes before induction of anesthesia in group B. Rats in group C were given no drugs before induction of anesthesia. Results: The intraoperative MAP, HR and SpO2 in group A were better than those in group B and C. The total dosage of propofol, VAS score and the number of physical activity in group A and group B were lower than those in group C, and the wake time of group A Lower than B, C two groups; B group respiratory depression was significantly higher than the number of A, C two groups. CONCLUSION: Both flurbiprofen axetil and fentanyl advanced analgesia are effective in painless abortion. However, the use of flurbiprofen axetil and fentanyl for respiratory inhibition is more suitable for painless Abortion.
