目的分析磁共振弥散加权成像(DWI)和弥散张量成像(DTI)对腰椎间盘早期退变的诊断价值。方法 80例无腰痛或腰椎部疾病的健康人,对L1~S1椎间盘行DWI与DTI扫描,生成表观弥散系数以及部分各项异性的伪彩图。在椎间盘髓核中放置感兴趣点,分别对ADC与FA值进行检测。结果分析影像学结果发现,相同椎间盘中不同区域的ADC与FA值比较差异具有统计学意义(P<0.05)。结论 ADC与FA值能够较为客观的表现出椎间盘水分子弥散能力,通过量化反映表现出椎间盘退变早期的状态,应用DWI与DTI扫描能够为早期椎间盘退变提供有效的诊断依据。
Objective To analyze the diagnostic value of diffusion weighted imaging (DWI) and diffusion tensor imaging (DTI) in the early degeneration of lumbar disc. Methods Eighty healthy volunteers without lumbago or lumbar vertebra disease were scanned with DWI and DTI on L1 ~ S1 intervertebral discs, and the apparent diffusion coefficients and some pseudo-color maps were generated. Points of interest were placed in the disc nucleus, ADC and FA values were detected. Results Analysis The imaging findings showed that there was significant difference between ADC and FA values in different regions of the same intervertebral disc (P <0.05). Conclusions ADC and FA values can objectively show the diffusivity of intervertebral disc water molecules. By quantitatively reflecting the state of early disc degeneration, using DWI and DTI scan can provide an effective diagnostic basis for early disc degeneration.
