目的 了解随肢体延长幅度的增加 ,由受牵拉的跨关节软组织增加的张力产生的关节面之间的压力对关节软骨的影响。方法 对 3组家兔分别进行不同幅度的胫骨延长 ,于牵开结束时观察膝关节软骨的组织病理学变化。结果 A组 (延长 15 % )软骨细胞增多伴空泡变性。B组 (延长2 5 % )出现裂隙 ,表层和移行层中软骨细胞片状坏死。C组 (延长 35 % )关节软骨部分或全层缺损。基质沙黄O染色浓度A组轻度丧失 ,C组严重丧失。Mallory染色显示胶原纤维 ,A组排列规则 ;B组增粗 ,走行紊乱 ;C组破坏严重。结论 肢体延长中 ,关节软骨可能发生压力坏死 ,其发生率及程度与关节软骨所受压力大小有关。延长幅度越大 ,压力会越大。
Objective To understand the effect of the pressure between the articular surfaces caused by the increased tension of the stretched cross-sectional soft tissue on the articular cartilage as the limb extension increases. Methods Three groups of rabbits were respectively subjected to different extensional tibial extension, and histopathological changes of knee joint cartilage were observed at the end of distraction. Results Group A (15% prolongation) increased chondrocytes with vacuolar degeneration. In Group B (25% extension), there were fissures, and chondrocyte lamellar necrosis was observed in the surface and transitional layers. Group C (extended 35%) of articular cartilage defects in part or full thickness. Substance Sand O stained slightly in group A and severe in group C. Mallory staining collagen fibers, A group of rules; B group thickening, walking disorder; C group of serious damage. Conclusion In the limb extension, the articular cartilage may undergo pressure necrosis. The incidence and degree of articular cartilage are related to the pressure on the articular cartilage. The larger the extension, the greater the pressure.
