目的:探讨磷酸西格列汀联合甘精胰岛素治疗老年2型糖尿病(T2DM)的疗效,为临床治疗提供相应依据。方法:收集宝鸡市中心医院2011年12月至2013年12月82例老年T2DM患者作为研究对象。随机分为对照组与观察组各41例。对照组给予甘精胰岛素治疗,观察组给予磷酸西格列汀联合甘精胰岛素治疗,观察并比较两组患者糖代谢指标、体重指数、胰岛功能及不良反应发生情况。结果:治疗后,观察组患者FBG、2 h PG和Hb A1c以及BMI均显著低于对照组[(5.2±1.5)vs(6.8±1.6)mmol/L,(8.3±1.6)vs(10.4±1.8)mmol/L,(6.4±1.1)vs(7.2±0.9)%,(20.1±1.8)vs(22.7±1.4)kg/m2],差异均具有统计学意义(P<0.05);观察组患者空腹C肽、餐后2 h C肽均显著高于对照组[(545.4±58.3)vs(516.7±51.4)pmol/L,(1 818.5±331.2)vs(1 532.2±298.0)pmol/L],差异均具有统计学意义(P<0.05)。治疗过程中,两组患者均未出现低血糖事件及其他严重不良反应。结论:磷酸西格列汀联合甘精胰岛素治疗老年T2DM疗效可靠,可以改善患者糖代谢、胰岛功能,并减轻体重,且不良反应少,临床上值得进一步研究。
Objective: To investigate the effect of sitagliptin phosphate and insulin glargine in the treatment of type 2 diabetes mellitus (T2DM) in elderly patients and provide the basis for clinical treatment. Methods: Eighty-two elderly patients with T2DM from December 2011 to December 2013 in Baoji Central Hospital were collected for the study. Randomly divided into control group and observation group, 41 cases each. The control group was given insulin glargine. The observation group was treated with sitagliptin phosphate and insulin glargine. The glucose metabolism index, body mass index, islet function and adverse reactions were observed and compared between two groups. Results: After treatment, FBG, 2 h PG, Hb A1c and BMI in observation group were significantly lower than those in control group [(5.2 ± 1.5) vs (6.8 ± 1.6) mmol / L, (8.3 ± 1.6) vs (6.4 ± 1.1 vs 7.2 ± 0.9)% and (20.1 ± 1.8) vs (22.7 ± 1.4) kg / m2, respectively, with significant differences between the two groups (P <0.05) C peptide, and postprandial 2-h C peptide were significantly higher than those in the control group [(545.4 ± 58.3 vs 516.7 ± 51.4 pmol / L, (1818.5 ± 331.2 vs vs 153.2 ± 298.0 pmol / L] All were statistically significant (P <0.05). During the course of treatment, no hypoglycemic events and other serious adverse events occurred in both groups. Conclusion: Sipagliptin phosphate combined with insulin glargine in the treatment of elderly patients with T2DM is reliable and can improve the patients’ glucose metabolism, islet function, weight loss and less adverse reactions, which is worth further study.
