目的探讨孕中期产前筛查中母血清AFP测定和产前超声诊断胎儿神经管缺陷畸形的价值。方法采用时间分辨荧光免疫分析法检测12 881例孕中期妇女血中AFP浓度,结合孕妇年龄、孕周、体重等参数,通过2T-Risks风险评估软件进行风险评估。对神经管缺陷(NTD)高风险者经三维彩超确诊。结果 12 881例孕妇中,神经管缺陷高风险者为78例,经超声确诊为神经管缺陷15例。结论孕中期血清学产前筛查结合产前超声诊断可有效地避免NTD缺陷儿的出生,是降低出生缺陷,实现优生优育的一种有效途径。
Objective To investigate the value of maternal serum AFP determination and prenatal ultrasound in the diagnosis of fetal neural tube defects in the second trimester of prenatal screening. Methods AFP concentrations in 12 881 pregnant women were detected by time-resolved fluorescence immunoassay. The risk of pregnancy was assessed by 2T-Risks risk assessment software based on parameters such as age, gestational age and weight of pregnant women. High risk of neural tube defects (NTD) confirmed by three-dimensional ultrasound. Results Among 12 881 pregnant women, 78 were high risk of neural tube defects and 15 were diagnosed as neural tube defects by ultrasound. Conclusion The prenatal screening combined with prenatal diagnosis can prevent the birth of NTD-deficient children effectively. It is an effective way to reduce birth defects and achieve prenatal and postnatal care.
