目的探讨热毒宁注射液治疗重症中暑的临床疗效及其对患者高凝状态的影响。方法选取攀枝花市中西医结合医院2012年1月—2015年1月收治的重症中暑患者60例,随机分为对照组和观察组,各30例。对照组患者给予0.9%氯化钠溶液治疗,观察组在对照组基础上给予热毒宁注射液治疗,两组患者均持续治疗7 d。比较两组患者治疗前后凝血功能指标[活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、纤维蛋白降解产物(FDP)、血小板计数(PLT)]及血液流变学指标[全血黏度、血浆黏度、血细胞比容(HCT)、红细胞变形指数、红细胞聚集指数]。结果治疗前,两组患者APTT、PT、FDP、PLT比较,差异无统计学意义(P>0.05);治疗后,观察组患者APTT、PT短于对照组,FDP低于对照组,PLT高于对照组(P<0.05)。治疗前,两组患者全血黏度、血浆黏度、HCT、红细胞变形指数、红细胞聚集指数比较,差异无统计学意义(P>0.05);治疗后,观察组患者全血黏度低切、中切、高切及血浆黏度、HCT、红细胞聚集指数低于对照组,红细胞变形指数高于对照组(P<0.05)。结论热毒宁注射液治疗重症中暑的效果明显,可改善患者全身高凝状态及血液流变性。
Objective To investigate the clinical efficacy of Renduining injection in treating severe heat stroke and its effect on hypercoagulability in patients. Methods Sixty patients with severe heat stroke admitted from January 2012 to January 2015 in Panzhihua Integrative Medicine Hospital were randomly divided into control group and observation group, with 30 cases in each group. Patients in the control group were treated with 0.9% sodium chloride solution. The observation group was given Radixin injection on the basis of the control group, and both groups were treated for 7 days. The indexes of coagulation function (APTT, PT, fibrin degradation product (FDP) and platelet count (PLT)) and hemorheological indexes Blood viscosity, plasma viscosity, hematocrit (HCT), erythrocyte deformability index, erythrocyte aggregation index]. Results Before treatment, there was no significant difference in APTT, PT, FDP and PLT between the two groups (P> 0.05). After treatment, the APTT and PT in the observation group were shorter than those in the control group, and the FDP was lower than that in the control group Control group (P <0.05). Before treatment, the whole blood viscosity, plasma viscosity, HCT, erythrocyte deformability index and erythrocyte aggregation index of two groups had no significant difference (P> 0.05). After treatment, the whole blood viscosity, High shear and plasma viscosity, HCT, erythrocyte aggregation index were lower than those in the control group, erythrocyte deformability index was higher than the control group (P <0.05). Conclusion Reveduning injection is effective in treating severe stroke, which can improve whole body hypercoagulability and hemorrheology.
