目的探讨抑癌基因p16、增殖细胞核抗原PCNA在膀胱肿瘤中的表达及其意义。方法应用免疫组化S-P方法和图像分析技术,对86例膀胱癌进行了抑癌基因p16、增殖细胞核抗原PC-NA标记和标记阳性部位的面积、积分光度值测定分析。结果p16表达程度随肿瘤的恶性度上升而降低,PCNA的表达程度随肿瘤的恶性程度上升而上升,p16、PCNA的表达与自然病程的早晚、长短,肿瘤的分期、分级及预后密切相关,P16低表达者PCNA高表达,肿瘤增生活跃,预后差,反之预后好。图像分析结果显示,病理分级越高p16表达的面积越小,积分光度值也呈减少趋势,PCNA表达的面积、积分光度值越高。随访资料表明:p16蛋白低表达、PCNA高表达者肿瘤复发率明显高于p16蛋白高表达和PCNA低表达者。结论对膀胱肿瘤进行p16、PCNA的检测可以作为肿瘤增殖活性、肿瘤分级、判断病人病程及预后的重要参考指标。
Objective To investigate the expression and significance of tumor suppressor gene p16 and proliferating cell nuclear antigen PCNA in bladder tumor. Methods The immunohistochemical S-P method and image analysis technique were used to analyze the area and integral photometric value of the tumor suppressor gene p16, proliferating cell nuclear antigen PC-NA and the positive mark in 86 cases of bladder cancer. Results The expression of p16 decreased with the malignant degree of the tumor. The expression of PCNA increased with the malignant degree of the tumor. The expression of p16 and PCNA was closely related to the course of the natural course, the length of the tumor, the stage of the tumor, the grade and the prognosis of P16 Low expression of PCNA expression, tumor hyperplasia, poor prognosis, whereas the good prognosis. The results of image analysis showed that the higher the pathological grade was, the smaller the area of p16 expression was and the lower the integral photometric value was. The higher the PCNA expression area and integral photometric value were. Follow-up data show that: p16 protein is low, high expression of PCNA tumor recurrence was significantly higher than p16 protein expression and PCNA low expression. Conclusion The detection of p16 and PCNA in bladder tumor can be used as an important reference index for tumor proliferation activity, tumor grade and judgment of patient’s disease course and prognosis.
