目的研究不同强度运动康复治疗对慢性心力衰竭患者心功能的影响,为临床心力衰竭患者早期开展康复治疗提供依据。方法选自2014年6月—2016月5月的漯河市医学高等专科学校第三附属医院120例慢性心力衰竭住院患者,随机分为三组:对照组40例,给予常规口服抗栓、利尿、扩血管药物治疗;观察A组40例,在上述常规治疗基础上进行中等强度康复治疗;观察B组40例,在上述常规治疗基础上进行高强度康复治疗。4周为一疗程,分别于入院时及疗程4周后结束时测定左室射血分数(LVEF)、心脏指数(CI)、6 min步行距离以及生活质量评定。采用SPSS 13.0统计软件进行统计分析,计量资料用均数±标准差(xˉ±s)表示,计数资料用率(%)表示,采用χ2检验,P<0.05为差异有统计学意义。结果4周疗程结束时,与对照组比较,康复治疗组的LVEF、CI、6 min步行距离及生活质量评定明显优于对照组,且观察B组较观察A组改善的更为明显,差异有统计学意义(P<0.05)。结论运动康复治疗对于慢性心力衰竭患者心功能的改善及生活质量的提高有肯定的作用。在患者可耐受的前提下,运动强度越大,心功能及生活质量的改善程度越大。
Objective To study the effect of sports rehabilitation therapy with different intensities on cardiac function in patients with chronic heart failure and provide evidence for the early rehabilitation of patients with clinical heart failure. Methods From June 2014 to May 2016, 120 hospitalized patients with chronic heart failure in the Third Affiliated Hospital of Luohe Medical College were randomly divided into three groups: control group, 40 patients were given conventional oral antithrombotic, diuretic, Vasodilator drug therapy; observed 40 cases in group A, and received moderate-intensity rehabilitation on the basis of the above routine treatment; observed 40 cases in group B and performed intensive rehabilitation on the basis of the conventional treatment. Four weeks for a course of treatment, left ventricular ejection fraction (LVEF), cardiac index (CI), walking distance 6 min and quality of life were measured at admission and 4 weeks after treatment. Statistical analysis was carried out by using SPSS 13.0 statistical software. The measurement data were expressed as mean ± standard deviation (xˉ ± s), and the count data was expressed as%, using χ2 test. P <0.05 was considered statistically significant. Results At the end of the 4-week course, the LVEF, CI, 6-minute walking distance and quality of life in the rehabilitation group were significantly better than those in the control group, and the improvement in group B was more obvious than in the control group Statistical significance (P <0.05). Conclusion Exercise rehabilitation therapy can improve cardiac function and improve quality of life in patients with chronic heart failure. Under the premise of patients can be tolerated, the greater the exercise intensity, the greater the improvement of cardiac function and quality of life.
