目的 观察地佐辛联合舒芬太尼用于神经外科术后患者自控静脉镇痛(PCIA)的效果.方法 择期行开颅神经外科手术患者90例随机均分为三组:DS组PCIA配方为舒芬太尼1.5μg/kg+地佐辛0.2mg/kg+托烷司琼0.2mg/kg,加生理盐水至100ml;S组为舒芬太尼2.0μg/kg+托烷司琼0.2mg/kg,加生理盐水至100ml;背景输注量2ml/h,单次追加量0.5ml,锁定时间15min.C组不使用PCIA.记录三组患者术后4、16、24和48h的VAS疼痛评分和Ramsay镇静评分,观察术后不良反应发生情况.结果 术后各时间点C组VAS疼痛评分高于S组和DS组(P<0.05).术后4h,DS组和S组镇静满意率高于C组(P<0.05).S组术后不良反应发生率高于DS组和C组(P<0.05).结论 地佐辛联合舒芬太尼用于术后PCIA,镇痛效果满意,不良反应发生率低.“,”Objective To observe the efficacy of patient-controlled intravenous analgesia(PCIA) with dezocine combined with sufentanil after neurosurgical operation.Methods A total of 90 patients underwent crainotomy was equally randomized into three groups.The patients in groups of DS and S accepted PCIA with sufentanil 1.5μg/kg+dezocine 0.2mg/kg+tropisetron 0.2mg/kg and sufentanil 2.0μg/kg+tropisetron 0.2mg/kg,respectively,which were diluted in normal saline 100ml.The analgesic variables were set up as a background infusion of 2ml/h,PCIA dose 0.5ml in a lockout interval of 15 minutes.The patients in group C did not use PCIA as the controls.The VAS pain and Ramsay sedation scores were evaluated at 4,16,24 and 48hours during PCIA.The postoperative adverse effects were observed.Results The VAS pain score in group C was higher than that in groups of S and DS at each time point(P<0.05).The satisfaction rate for sedation at 4 hours after operation was higher in groups of DS and S than that in group C(P<0.05).The incidence of postoperative adverse effects in group S was higher than that in groups of DS and C(P<0.05).Conclusion PCIA with dezocine and sufentanil has satisfactory analgesic effect and less adverse effects.
