目的探讨组织金属蛋白酶抑制剂1(TIMP-1)与基质金属蛋白酶9(MMP-9)在胆囊癌及癌前病变组织中的表达,阐明其与胆囊癌发生、发展的关系。方法应用免疫组化SP法检测TIMP-1和MMP-9在42例胆囊癌、18例胆囊上皮中重度不典型增生、13例胆囊上皮轻度不典型增生及30例慢性胆囊炎组织中的表达。结果胆囊癌组中MMP-9的阳性表达率(88.10%)较轻度不典型增生组(30.77%)及慢性胆囊炎组(20.00%)高,差异有统计学意义(P<0.0083),其表达强度差异亦有统计学意义(P<0.0083)。胆囊癌组TIMP-1的阳性表达率(90.48%)高于慢性胆囊炎组(43.33%),差异有统计学意义(P<0.0083),其表达强度差异亦有统计学意义(P<0.0083)。不同胆囊病变组织中MMP-9和TIMP-1的表达呈正相关(r=0.222,P=0.024)。慢性胆囊炎,不典型增生和胆囊癌组中的MMP-9/TIMP-l的值存在差异,差异有统计学意义(χ2=6.851,P=0.033)。结论 MMP-9和TIMP-1的阳性表达率及强度与胆囊癌的发生、发展有关,二者的比值可用来评估胆囊癌的预后。
Objective To investigate the expression of tissue inhibitor of metalloproteinase-1 (TIMP-1) and matrix metalloproteinase-9 (MMP-9) in gallbladder carcinomas and precancerous lesions and to elucidate their relationship with the occurrence and development of gallbladder carcinomas. Methods The expressions of TIMP-1 and MMP-9 in 42 cases of gallbladder carcinoma, 18 cases of moderate and severe gallbladder epithelial dysplasia, 13 cases of mild gallbladder epithelial dysplasia and 30 cases of chronic cholecystitis were detected by immunohistochemical SP method . Results The positive expression rate of MMP-9 in gallbladder carcinoma group was significantly higher than that in mild dysplasia group (30.77%) and chronic cholecystitis group (88.00%), the difference was statistically significant (P <0.0083) The difference in expression intensity was also statistically significant (P <0.0083). The positive expression rate of TIMP-1 in gallbladder cancer group was higher than that in chronic cholecystitis group (90.3% vs 43.33%, P <0.0083), and the difference was also statistically significant (P <0.0083) . The expression of MMP-9 and TIMP-1 in different gallbladder lesions was positively correlated (r = 0.222, P = 0.024). The values of MMP-9 / TIMP-1 in chronic cholecystitis, atypical hyperplasia and gallbladder carcinoma were significantly different (χ2 = 6.851, P = 0.033). Conclusions The positive expression rate and intensity of MMP-9 and TIMP-1 are related to the occurrence and development of gallbladder carcinoma. The ratio between them can be used to evaluate the prognosis of gallbladder carcinoma.
