目的评价儿童不同剂次乙肝疫苗加强免疫后的中短期效果,为儿童加强免疫提供依据。方法随机选择1周岁内完成乙肝疫苗基础免疫但无乙肝疫苗加强免疫史的5~14岁玉环县户籍儿童。乙肝表面抗体(抗-HBs)阳性儿童加强免疫1剂乙肝疫苗,抗-HBs阴性儿童加强免疫3剂乙肝疫苗,对抗-HBs阳性儿童加强免疫前、加强免疫1剂后6个月及18个月,抗-HBs阴性儿童加强免疫前、加强免疫3剂后1个月及12个月进行采血并定量检测抗-HBs水平。结果抗-HBs阳性儿童加强免疫前、加强免疫1剂后6个月及18个月的抗体平均浓度分别为463.38、3 666.90和1 425.19 mIU/mL,加强1剂免疫后6个月及18个月的抗体平均浓度与加强免疫前比较,差异均有统计学意义(P<0.05)。抗-HBs阴性儿童加强免疫前、加强免疫3剂后1个月及12个月的抗体平均浓度分别为2.63、3 479.24和475.08 mIU/mL,加强免疫3剂后1个月及12个月的抗体平均浓度与加强免疫前比较,差异均有统计学意义(P<0.05)。不同疫苗加强免疫后18个月的抗体平均浓度与加强免疫前比较,差异均有统计学意义(P<0.05)。结论 5~14岁儿童中开展乙肝疫苗加强免疫效果良好,免疫后18个月抗体水平基本维持在强阳性水平。
