目的:临床中约有70%的脑卒中患者存在着不同程度的功能障碍,其中失语症影响了患者的社会交往能力,甚至给社会和家庭带来沉重的经济负担。本文综述了有关失语症恢复机制的文献,力求为进一步的研究提供借鉴。资料来源:应用计算机检索Medline1980-01/2004-01期间的相关文章,检索词“aphasia,recovery”,“language,recovery”,并限定文章语言种类为English。资料选择:对资料进行初审,选取有关失语症恢复机制的相关文献,然后筛除明显不随机临床试验的研究,对剩余的文献查找全文,进一步判断是否为随机对照实验。纳入标准为随机对照临床研究,无论是否为盲法。资料提炼:共收集到22篇关于失语症恢复机制的相关文献,12篇符合纳入标准。排除的10篇试验均为重复的同一研究。资料综合:语言功能的恢复与性别、年龄、受教育程度、病变性质、病变范围、发病到语言康复的时间及患者语言障碍的自知力、自我纠正能力等因素有关,因此,以往的有关失语症恢复机制的研究各有不同的发现和观点。结论:关于失语症的恢复机制,目前尚无统一的结论。脑语言功能的可塑性可能为失语症的恢复机制。
OBJECTIVE: About 70% of patients with stroke in clinic have different levels of dysfunction. Among them, aphasia affects the social interaction ability of patients and even brings a heavy financial burden to society and families. This article reviews the literature on recovery mechanisms of aphasia and seeks to provide a source for further research. Source: The related articles from Medline1980-01 / 2004-01 were retrieved by computer. The terms “aphasia, recovery”, “language, recovery” were searched, and the article language was limited to English. Data selection: The data were first reviewed, relevant literature on recovery mechanism of aphasia was selected, and then the study of obviously nonrandomized clinical trials was screened. The remaining literature was searched for the full text to further determine whether it was a randomized controlled trial. Inclusion criteria were randomized controlled clinical studies, whether blinded or not. Data Extraction: A total of 22 articles on aphasia recovery mechanisms were collected and 12 articles met the inclusion criteria. The 10 excluded trials were all duplicates of the same study. DATA SYNTHESIS: The restoration of language function is related to factors such as sex, age, education level, the nature of the lesion, the extent of the lesion, the time from onset to language rehabilitation and patient’s language barrier, self-correcting ability and so on. Therefore, Research on the mechanism of recovery have different findings and perspectives. Conclusion: There is no unified conclusion about the mechanism of aphasia recovery. The plasticity of brain language function may be a recovery mechanism of aphasia.
