目的:观察固胎茵陈汤预防ABO母儿血型不合的临床疗效。方法:IgG抗A(抗B)抗体效价>1∶128的孕妇85例,随机分为3组,用药组32例,西药组28例,中药组25例。西药组予维生素C、维生素E及鲁米那口服;中药组口服茵陈蒿汤,用药组口服固胎茵陈汤。观察IgG抗A(B)抗体效价、新生儿溶血指标、阿普加(Apga)r评分、体重及黄疸开始时间、黄疸程度等。结果:总有效率用药组为93.75%,西药组为75.00%,中药组为96.00%,用药组与西药组比较,差异有显著性意义(P<0.05);用药组与中药组比较,差异无显著性意义(P>0.05)。用药组与中药组新生儿脐血总胆红素均低于西药组,差异均有显著性意义(P<0.05)。而3组新生儿Apgar评分、体重、脐血红蛋白比较,差异无显著性意义(P>0.05)。用药组新生儿有2例发生早发性黄疸,1例重度黄疸,无溶血病例;西药组新生儿有5例发生早发性黄疸,3例重度黄疸,2例发生溶血病;中药组新生儿有1例发生早发性黄疸,1例重度黄疸,无溶血病发生。西药组发生黄疸的情况明显多于用药组及中药组,差异均有显著性意义(P<0.05)。结论:固胎茵陈汤防治ABO母儿血型不合有较好的疗效,能有效降低新生儿ABO血型不合引起的黄疸发病率。
Objective: To observe the clinical efficacy of Fuzhanyin Decoction in preventing ABO mother-child blood group incompatibility. Methods: 85 pregnant women with antibody titer of IgG anti-A (anti-B) antibody> 1:128 were randomly divided into 3 groups: 32 cases in medication group, 28 cases in western medicine group and 25 cases in traditional Chinese medicine group. Western medicine group to vitamin C, vitamin E and luminal that oral; Chinese medicine group oral Yin Chen Hao decoction, medication group oral solid fetal Yin Chen Tang. The antibody titer of IgG anti-A (B), neonatal hemolytic index, Apga r score, body weight, jaundice start time and jaundice degree were observed. Results: The total effective rate was 93.75% in the medication group, 75.00% in the western medicine group and 96.00% in the traditional Chinese medicine group. There was significant difference between the medication group and the western medicine group (P <0.05). There was no significant difference between the medication group and the traditional Chinese medicine group Significant significance (P> 0.05). Neonatal cord blood total bilirubin in the medication group and the traditional Chinese medicine group were lower than those in the western medicine group, with significant difference (P <0.05). There was no significant difference in Apgar score, body weight and cord hemoglobin between the three groups (P> 0.05). There were 2 cases of early onset jaundice, 1 case of severe jaundice and no hemolytic cases in the newborn infants of the medication group, 5 cases of early-onset jaundice, 3 cases of severe jaundice and 2 cases of hemolytic disease in the newborn of the western medicine group. 1 case of early onset jaundice, 1 case of severe jaundice, no hemolytic disease. Western medicine group jaundice was significantly more than the drug group and the Chinese medicine group, the difference was significant (P <0.05). CONCLUSION: Guanyin Yinchen decoction can prevent and treat ABO maternal blood type incompatibility effectively and effectively reduce the incidence of jaundice caused by neonatal ABO blood group incompatibility.
