小儿急性和慢性上颌窦炎的发病率较高,保守治疗仍未取得很满意的效果。作者以前发现,大部分急性和慢性上颌窦炎患者存在着局部免疫缺陷,表现为上颌窦分泌性免疫球蛋白A(SIgA)合成能力降低或缺陷,提示人们使用免疫剂来矫正其局部免疫功能。为此,作者探讨使用左旋咪唑和初乳对上颔窦炎患儿进行局部免疫治疗。分三组对比观察,182例小儿上颌窦炎患者(急性100例,慢性急性发作期82例)使用免疫疗法的效果,对照组67例,其中39例为上颌窦分泌物中SIgA含量降低或缺乏,28例含量正常。发现SIgA降低者上颌窦穿剂次数和住院天数较SIgA正常者显著延长,左旋咪唑治疗组85例,其中40例SIgA降低或缺乏,使用0.2~0.5%带抗生素的左旋咪唑溶液注入上颌窦冲洗。结果发现SIgA降低患者住院时间显著缩短。初乳治疗组30例,收集健康产妇5天内的初乳注入冲洗后的上颌窦,每次每侧剂量为:<5岁1ml,5~
The incidence of acute and chronic pediatric maxillary sinusitis is high, and conservative treatment has not yet achieved satisfactory results. The authors previously found that most patients with acute and chronic maxillary sinusitis present with localized immunodeficiency as evidenced by reduced or defective synthesis of SIgA in the maxillary sinus, prompting the use of immunologic agents to correct their local immune function. To this end, the authors discuss the use of levamisole and colostrum in the treatment of infantile sinusitis in children with local immunotherapy. Divided into three groups, 182 children with maxillary sinusitis (acute 100 cases, 82 cases of chronic acute exacerbation of the use of immunotherapy, 67 cases in the control group, of which 39 cases of maxillary sinus secretion SIgA decreased or lack of , 28 cases normal content.
