目的评价双微管技术弹簧圈栓塞颅内宽颈动脉瘤的影像及临床疗效。方法选取2013年1月至2015年1月收治的颅内宽颈动脉瘤的患者,首先尝试应用双微管技术进行栓塞,如不能形成稳定的框架结构则改为支架或者球囊辅助栓塞。其中42例患者使用双微管技术成功栓塞。结果术后即时脑血管造影显示完全栓塞22例,近全栓塞14例,部分栓塞6例。出院时格拉斯哥预后评分5分20例,4分10例,3分7例,2分3例,1分(死亡)2例。术后35例随访3~12个月,无再出血病例,28例复查脑血管造影显示动脉瘤相对复发2例,使用支架辅助技术再次栓塞。结论双微管技术弹簧圈栓塞颅内宽颈动脉瘤是一种相对安全有效的治疗方法。
Objective To evaluate the imaging and clinical efficacy of double microtubule technique in the treatment of intracranial wide-necked aneurysm with coil embolization. Methods From January 2013 to January 2015, patients with wide-necked intracranial aneurysms were initially treated with dual-microtubules for embolization. Stent or balloon-assisted embolization was used if they did not form a stable framework. Forty-two of these patients successfully embolized using dual-microtubule techniques. Results Immediate postoperative cerebral angiography showed complete embolism in 22 cases, nearly complete embolization in 14 cases and partial embolization in 6 cases. At discharge, Glasgow’s prognosis score was 5 points in 20 cases, 4 points in 10 cases, 3 points in 7 cases, 2 points in 3 cases and 1 point (death) in 2 cases. 35 cases were followed up for 3 to 12 months, no rebleeding cases, 28 cases of recurrent cerebral angiography showed aneurysm relapse in 2 cases, the use of stent-assisted technique again embolization. Conclusion The double microtubule technique is a relatively safe and effective treatment for intracranial wide-necked aneurysm with coil embolization.
