目的探讨老年左半结肠癌急性梗阻一期切除吻合的方法及临床效果。方法回顾性分析2007年5月至2012年5月本院经一期切除吻合手术治疗的12例老年左半结肠癌急性梗阻患者的临床资料。结果 12例患者均结肠灌洗后一期手术治疗。手术时间(153±16)min,结肠灌洗时间56±9min;根治性切除7例,姑息手术5例,术后切口感染2例,未出现吻合口瘘等并发症。结论老年左半结肠癌急性梗阻全结肠灌洗、一期切除避免了结肠造瘘及二次手术,肿瘤根治机会增加,住院时间缩短,患者生存质量改善,临床应严格适应症选择。
Objective To investigate the method and clinical effect of primary resection and anastomosis of acute obstruction of left-sided colon cancer in the elderly. Methods The clinical data of 12 patients with acute obstruction of left colon cancer who underwent primary resection and anastomosis in our hospital from May 2007 to May 2012 were retrospectively analyzed. Results All 12 patients underwent colonic lavage surgery. The operative time (153 ± 16) min and colonic lavage time were 56 ± 9 min. Seven patients underwent radical resection and 5 patients underwent palliative surgery. Two patients had incision infection after operation, and no complications such as anastomotic fistula occurred. Conclusion Acute obstruction of the left colon cancer in the whole colon lavage, a resection to avoid colostomy and secondary surgery, tumor radical opportunity to increase, shorter hospital stay, patients with improved quality of life, clinical indications should be strictly chosen.
