本文通过对27例中度高原的西宁地区39~42孕周胎盘绒毛超微结构的观察,并与南京平原组相应孕周胎盘作比较。结果提示:西宁地区胎盘绒毛滋养细胞和间质在40孕周已有改变。41孕周合体细胞表面微绒毛减少,粗面内质网扩张空泡变,饮液泡减少,细胞滋养细胞增生,易见中间型细胞。间质滋养层基底膜持续增厚及胶原纤维增多、血管内皮细胞皱缩,管腔狭窄等一系列老化的形态学改变与平原组42孕周(过期妊娠)胎盘相同。表明高原西宁地区胎盘提前发生老化,并认为可能与高原低压氧等自然环境有关,据此提出,西宁地区应在40孕周时实施计划分娩,以降低围产儿死亡率及并发症。
In this paper, 27 cases of moderate plateau in Xining area 39 ~ 42 gestational weeks placental ultrastructure was observed and compared with the corresponding placental placenta in Nanjing plain group for comparison. The results suggest that: placental trophoblast trophoblast and stroma in Xining have changed in 40 weeks. 41 weeks gestational weeks combined with reduced microvilli on the surface cells, rough endoplasmic reticulum expansion vacuolar change, reducing the number of vacuoles, cytotrophoblastic proliferation, easy to see the intermediate cells. Mesenteric trophoblast continued thickening of the basement membrane and collagen fibers, vascular endothelial cell shrinkage, stenosis and a series of morphological changes of the placental group 42 gestational weeks (overdue pregnancy) the same placenta. Indicating that placental premature aging occurs in Xining Plateau and may be related to the natural environment such as plateau hypobaric oxygen. Therefore, it is suggested that planned delivery should be implemented in Xining during the 40th week to reduce perinatal mortality and complications.
