目的:研究儿童传染性单核细胞增多症(Infectious mononucleosis,IM)通过中、西医及中西医结合三种不同的治疗方案进行比较,进而得出更适合本病的治法。方法:前瞻性研究我科住院IM患儿68例并随机分为西药组、中药组和中西药结合组,西药组予静脉点滴更昔洛韦,中药组予中药辨证施治,中西药结合组在西药治疗基础上加用中药辨证治疗。结果:通过对中药组、西药组和中西药结合组三种治疗方案的对比,发现三组总有效率在统计学比较中并无显著性差异。在退热时间、咽充血持续时间、淋巴结肿大持续时间等临床症状的消退方面的研究中发现中药组优于西药组(更昔洛韦组),中药与中西药组间比较没有差异。同时在肝功能异常恢复的时间的比较中,中药组更占优势。结论:中医药对于IM的治疗是有效的,即准确的IM分期辨证治疗,可以较快的缩短临床症状体征,以及时截断病程。故临床治疗中要及时发现疾病发展过程中的不同阶段的中心证候进行辨证施治,不能一方一法治疗到底。只要辨证准确、方药得法,在治疗过程中中药将起到至关重要、事半功倍的效果。
Objective: To study the comparison of three different treatment regimens of Infectious mononucleosis (IM) in children with Chinese, Western and traditional Chinese and Western medicine so as to obtain more suitable treatment for this disease. Methods: A prospective study of 68 inpatients with IM in our department was randomly divided into western medicine group, traditional Chinese medicine group and traditional Chinese and western medicine combination group, western medicine group was given intravenous ganciclovir, traditional Chinese medicine group was treated with traditional Chinese medicine syndrome differentiation, On the basis of Western medicine plus traditional Chinese medicine syndrome differentiation treatment. Results: Comparing the three treatment regimens of traditional Chinese medicine group, western medicine group and traditional Chinese medicine combined with western medicine group, the total effective rate of the three groups showed no significant difference in statistical comparison. Traditional Chinese medicine group was superior to Western medicine group (ganciclovir group) in the study of clinical symptoms such as fever time, duration of pharyngeal congestion and duration of lymph node enlargement. There was no difference between TCM and western medicine group. At the same time in the recovery of liver dysfunction time comparison, the Chinese medicine group more dominant. Conclusion: Chinese medicine is effective for the treatment of IM, that is, accurate differential diagnosis of IM staging can shorten the clinical symptoms and signs, and timely cut off the course. Therefore, clinical treatment should be timely found in the disease development process of different stages of the syndromes of syndrome differentiation, can not be a treatment in the end. As long as the syndrome differentiation is accurate and the prescription is correct, traditional Chinese medicine will play an important and multiplying effect in the course of treatment.
