目的:分析不同绒毛膜性双胎妊娠的结局。方法:抽取2013年6月至2019年6月于郑州大学第一附属医院分娩双胎资料完整的产妇618例,依据绒毛膜性分为双绒双羊(DCDA)、单绒双羊(MCDA)、单绒单羊(MCMA)和绒毛膜性不详者。分析DCDA、MCDA者围生期情况。结果:618例双胎中,DCDA 449例,MCDA 137例,MCMA 1例,绒毛膜性不详31例。618例双胎孕妇中,早产473例(76.54%),妊娠期高血压疾病183例(29.61%)。MCDA孕妇早产、妊娠期高血压疾病、产后出血、羊水过少的发生率较高(χn 2=4.392、5.946、4.433、4.534,n P=0.036、0.015、0.035、0.033);DCDA孕妇胎膜早破、甲状腺功能减退发生率较高(χn 2=5.013、5.741,n P=0.025、0.018)。存活儿中入住NICU 588例,死亡23例,新生儿畸形13例。MCDA新生儿入住新生儿重症监护室、新生儿窒息、新生儿死亡发生率高于DCDA新生儿(χn 2=11.517、8.894、18.013,n P=0.001、0.003、0.001)。n 结论:与DCDA比较,MCDA孕妇早产和妊娠期高血压、胎儿不良结局发生风险更高。“,”Objective:To analyze the outcome of different chorionic twin pregnancy.Methods:A total of 618 parturients who gave birth to twins in the First Affiliated Hospital of Zhengzhou University from June 2013 to June 2019 were selected. They were divided into dichorionic diamnionic (DCDA), monochorionic diamnionic (MCDA), monochorionic monoamniotic (MCMA), and those with unidentified chorionicity. The perinatal situation of DCDA and MCDA were analyzed.Results:Among 618 cases of twins, 449 cases were DCDA, 137 cases were MCDA, 1 case was MCMA, and 31 cases were unidentified chorionicity. Among the 618 cases of twin pregnancy, born prematurely were 473 cases (76.54%), and hypertensive during pregnancy were 183 cases (29.61%). MCDA pregnant women had a higher incidence of premature delivery, hypertension in pregnancy, postpartum hemorrhage, and oligohydramnios (χn 2=4.392, 5.946, 4.433, 4.534, n P=0.036, 0.015, 0.035, 0.033); while the incidence of premature rupture of membranes and hypothyroidism in pregnant women with DCDA was higher (χn 2=5.013, 5.741, n P=0.025, 0.018). Among the surviving children, 588 cases were admitted to NICU, 23 cases died, and 13 cases were neonatal deformities. The incidence of neonatal intensive care, neonatal asphyxia and neonatal death in MCDA neonates were higher than those of DCDA neonates (χn 2=11.517, 8.894, 18.013, n P=0.001, 0.003, 0.001).n Conclusions:Compare with DCDA, MCDA pregnant women are at higher risk of premature delivery, hypertension pregnancy, and adverse fetal outcomes.
