目的探讨枪伤前后猪脑功能PET显像及各重要脏器病理学的改变。方法采用81式自动步枪,口径7.62mm,560式子弹,选取成年健康家猪左后肢肌肉丰厚处,避开大血管和骨头,射击距离5m,射击后立即止血包扎,1h后行脑功能PET检查,6h后进行脑、心、肝、脾、肾和肺等脏器的病理学检查。结果枪伤前后脑功能葡萄糖代谢有明显差异(P<0.01)。枪伤后除肺脏有病理学改变外,其他重要脏器无明显病理学改变。肺组织主要表现为肺泡萎陷,含气量减少伴炎细胞浸润,肺水肿,肺泡结构不清,小血管淤血等。结论枪伤后猪脑PET功能显像代谢明显增加,肺部损伤明显,其作用机制与应激、局部周围组织炎性反应和远达效应有关。
Objective To investigate the changes of pig brain functional PET imaging and pathological changes of vital organs before and after gunshot wound. Methods The muscle of the left hind limbs of adult healthy domestic pigs was selected with the automatic guns of type 81 and caliber 7.62mm and 560 bullets to avoid large blood vessels and bones. The firing distance was 5m. , 6h after the brain, heart, liver, spleen, kidney and lung and other organs pathological examination. Results There was a significant difference in brain glucose metabolism before and after gunshot wound (P <0.01). In addition to pathological changes in the lungs after gunshot wounds, other important organs without significant pathological changes. Lung tissue is mainly manifested as alveolar collapse, decreased gas content with inflammatory cell infiltration, pulmonary edema, alveolar structure, small blood vessels congestion and so on. Conclusions After gunshot wound, PET functional imaging metabolism of porcine brain is obviously increased and lung injury is obvious. The mechanism of action is related to stress, inflammatory reaction and teleconnection of local peripheral tissues.
