目的探讨依赖利福平结核分枝杆菌(简称依R菌)Rv0341抗体阳性初治肺结核患者的临床特点。方法纳入42例依R菌Rv0341抗体阳性的初治肺结核患者作为研究组,45例依R菌Rv0341抗体阴性的初治肺结核患者作为对照组,对两组的临床症状、X线影像学表现、痰结核分枝杆菌和依R菌培养阳性率、治疗效果等方面进行对比分析。结果研究组咳痰、咯血、气促的发生率明显高于对照组(P<0.05);X线影像学表现研究组病变更多累及双肺及多个肺野(P<0.05),对照组病变更多局限于单肺和一个肺野(P<0.05),且研究组出现空洞的比率高于对照组(P<0.05);在痰结核分枝杆菌培养阳性率上两组差异无统计学意义(P>0.05),但研究组痰依R菌培养阳性率组明显高于对照组(P<0.05);对照组的治愈、好转率高于研究组(P<0.05),对照组治疗无效的比例也低于研究组。结论依R菌Rv0341抗体阳性初治肺结核患者具有临床症状重,病变范围广,易出现空洞,依R菌培养阳性率较高,治疗效果欠佳的特点。
Objective To investigate the clinical features of newly diagnosed pulmonary tuberculosis (TB) patients with Rv0341 antibody positive for rifampicin-resistant Mycobacterium tuberculosis (R) strain. Methods Forty-two newly diagnosed pulmonary tuberculosis patients were enrolled in this study. Forty-five newly diagnosed pulmonary tuberculosis patients who were negative according to R bacterium Rv0341 antibody were used as the control group. Clinical symptoms, radiographic findings, sputum Mycobacterium tuberculosis and according to R bacteria culture positive rate, the treatment effect and so on comparative analysis. Results The incidence of sputum, hemoptysis and shortness of breath in the study group was significantly higher than that in the control group (P <0.05). The X-ray imaging findings of the study group more frequently affected the lungs and lung fields (P <0.05) The lesions were more confined to one lung and one lung field (P <0.05), and the ratio of cavities in the study group was higher than that in the control group (P <0.05). There was no significant difference in the positive rates of sputum Mycobacterium tuberculosis culture between the two groups (P> 0.05), but the sputum positive rate of sputum culture group was significantly higher than that of the control group (P <0.05); the cure rate of the control group was higher than that of the study group (P <0.05), and the control group was ineffective Also lower than the study group. Conclusion According to R bacteria Rv0341 positive tuberculosis patients with newly diagnosed patients with clinical symptoms, wide range of lesions, prone to empty, according to R bacteria culture positive rate is higher, the treatment effect is poor.
