目的:近年胃粘膜肠化与胃癌的关系引起人们的广泛重视,本文进一步探讨了二者之间的内在联系。方法:采用 ABC 免疫组织化学技术对230例肠化及胃癌组织中双花扁豆凝集素(DBA)受体的表达进行观察。根据粘液组织化学染色将胃癌分为肠型及胃型等,将肠化分为Ⅰ、Ⅱ及Ⅲ三型。结果:Ⅲ型肠化在肠型胃癌旁肠化及异型增生伴肠化的检出率均显著高于胃型胃癌旁肠化及胃炎伴肠化(P 均<0.01);肠型胃癌的 DBA 受体表达率及硫酸粘液检出率均显著高于胃型胃癌(P 均<0.01);Ⅲ型肠化 DBA 受体表达率显著高于Ⅰ、Ⅱ型肠化(P<0.05～0.01)。结论:Ⅲ型肠化与肠型胃癌关系密切,加强对Ⅲ型肠化的密切随访,对胃癌的早期发现可能具有重要意义。
Objective: In recent years, the relationship between gastric mucosal intestinal metastasis and gastric cancer has attracted widespread attention. This article further explores the intrinsic link between the two. Methods: ABC immunohistochemical technique was used to observe the expression of DBA receptor in 230 cases of intestinal metastasis and gastric cancer. According to the mucinous histochemical staining, gastric cancer is divided into intestinal type and gastric type, and the intestinal metaplasia is divided into three types: I, II and III. Results: The detection rate of type III intestinal metastasis was significantly higher in intestinal paragastric intestinal metastasis and dysplasia with intestinal metaplasia than in gastric adenocarcinoma of stomach type and gastritis with intestinal metaplasia (P <0.01); DBA of intestinal type gastric carcinoma The receptor expression rate and the detection rate of sulfuric acid mucus were significantly higher than that of gastric cancer (P <0.01). The expression rate of type III DBA receptor in intestinal metaplasia was significantly higher than that of type I and II (P <0.05-0.01). Conclusion: Type III intestinal metastasis is closely related to intestinal type gastric cancer. It is important to strengthen the close follow-up of type III intestinal metastasis, which may be of great significance for the early detection of gastric cancer.
