目的 探讨被动吸烟对冠状动脉搭桥术前大隐静脉桥血管中基质金属蛋白酶及其组织型抑制剂基因表达的影响.方法 按入选标准选择258例老年女性,拟行冠状动脉搭桥患者进行研究,按术前吸烟状态分为被动吸烟组87例,对照组105例,主动吸烟组66例.收集整理所有入选病人详细术前临床资料.术中收集大隐静脉标本,以real-time RT-PCR、免疫组化和western-blot方法阐明被动吸烟对冠状动脉搭桥术前大隐静脉桥血管中MMP-2、MMP-9、TIMP-1、TIMP-2的基因表达情况.结果 三组术前临床资料无统计学差异.与对照组相比,被动吸烟组和主动吸烟组大隐静脉桥血管中MMP-2和MMP-9基因表达明显增强,P<0.05,并以主动吸烟组更甚.与MMPs基因表达情况相反,TIMP-1、TIMP-2 mRNA的表达在被动吸烟组和主动吸烟组中均有降低,分别P<0.05,也同样以吸烟组更甚,P <0.05.结论 被动吸烟作为心血管病明确的独立危险因素,严重影响冠状动脉搭桥术前大隐静脉桥血管中MMPs/TIMPs基因表达平衡,并且这种平衡的打破,将对大隐静脉血管中细胞外基质的构成产生重要影响,进而影响静脉桥血管的通畅率.“,”Objective Passive smoking (PS) appears to increase the risks of coronary artery disease (CAD).Previous study has found that heavy smoking increases native matrix metalloproteinases-2 (MMP-2) and MMP-9 gene expression in saphenous vein (SV) before coronary artery bypass graft (CABG).However,little is known about the impacts of SHS on the characteristics of native MMPs gene expression in SV conduits before operation in retired Chinese women who were lifelong never-smokers.Methods Two hundred and fifty-eight patients were divided into 3 groups:PS,control and active smoking (AS) group,mRNA and protein levels of MMP-2,MMP-9,tissue inhibitors of metalloproteinase-l(TIMP-1) and TIMP-2 were analyzed.Results Compared with control group,MMP-2 and MMP-9 gene expression were significantly increased in the other two groups,respectively,P < 0.05.In contrast to MMPs response,TIMP-1 and TIMP-2 gene expression was significantly decreased,respectively,P<0.05.Conclusion Our data revealed the association between PS and the native characteristics of MMPs/TIMs gene expression in SV conduits.Furthermore,the dysregulated MMPs/TIMs gene expression has showed signs of the vein graft disease process before coronary surgery.
