本文对20例正常人(对照组)血清和42例分泌性中耳炎(SOM)患者(SOM组)中耳积液(MEF)和血清中可溶性白细胞介素2受体(sIL-2R)和纤维结合蛋白(Fn)水平进行了检测。结果显示:①SOM组血清sIL-2R含量明显高于对照组,而Fn含量低于对照组(P<0.01)。②MEF中IL-2R含量明显高于同组血清含量(P<0.01);Fn含量低于血清含量(P<0.05)。③鼻咽癌患者血清及MEF中sIL-2R含量明显高于一般SOM患者(P<0.01);Fn水平低于一般SOM患者(P<0.05)。④中耳粘液组sIL-2R水平高于浆液组,而Fn含量低于浆液组(P<0.001)。⑤慢性SOM的MEF中其sIL-2R含量更高,而Fn则相反。提示:①血清及MEF中sIL-2R和Fn含量测定有助于对SOM患者免疫状态的评估。②MEF中sIL-2R可能主要由局部中耳粘膜产生,在SOM炎症反应中起免疫抑制作用。③MEF中高浓度sIL-2R的存在和Fn的低水平可能是SOM迂延不愈的一个原因。
In this paper, 20 cases of normal (control group) serum and 42 cases of secretory otitis media (SOM) patients (SOM group) middle ear effusion (MEF) and serum soluble interleukin 2 receptor (sIL-2R) and fiber binding Protein (Fn) levels were tested. The results showed that: (1) The level of sIL-2R in SOM group was significantly higher than that in control group, while Fn content was lower than that in control group (P <0.01). ② IL-2R level in MEF was significantly higher than that in the same group (P <0.01); Fn content was lower than serum level (P <0.05). (3) The serum sIL-2R levels in patients with nasopharyngeal carcinoma and MEF were significantly higher than those in patients with normal SOM (P <0.01). The level of Fn in patients with nasopharyngeal carcinoma was lower than that in patients with SOM (P <0.05). The level of sIL-2R in middle ear mucus group was higher than that of serous group, while Fn content was lower than that of serous group (P <0.001). ⑤ chronic SOM MEF its sIL-2R content is higher, while Fn is the opposite. Tip: ① Serum and MEF sIL-2R and Fn content determination will help to assess the immune status of SOM patients. ② MEL in sIL-2R may be mainly produced by the local middle ear mucosa, SOM inflammatory response from the immunosuppressive effect. ③ The presence of high concentrations of sIL-2R in MEFs and the low level of Fn may be one of the reasons for the uncooperative SOM.
