目的:研究腹腔镜全子宫切除术(laparoscopy total hysterectomy,LTH)对机体免疫功能的影响程度。方法:选择因良性疾病需行LTH术的患者32例作为研究对象(LTH组);对照组为同期行开腹子宫切除术(abdominal total hysterectomy,ATH)且条件相似的28例患者(ATH组)。测定两组术前1天和术后1、4天血清IL-2、IL-10表达水平。结果:①两组术后1天外周血IL-2水平显著低于术前(均P<0.05),但ATH组下降程度明显大于LTH组(P<0.05);ATH组术后4天外周血IL-2水平较术后1天有所升高,但仍显著低于术前(P<0.05),LTH组术后4天IL-2回升至术前水平(P>0.05),与ATH组相比差异有统计学意义(P<0.05)。②ATH组术后1天外周血IL-10含量显著高于术前(P<0.05),术后4天有所下降,但仍明显高于术前(P<0.05);LTH组术后1、4天外周血IL-10较术前有所升高,但差异无统计学意义(P>0.05),与ATH组相比差异有统计学意义(P<0.05)。结论:LTH与ATH相比,创伤轻,对机体免疫功能影响小。
Objective: To investigate the effect of laparoscopy total hysterectomy (LTH) on immune function. Methods: Twenty-two patients (LTH group) undergoing LTH for benign diseases were selected as the control group. Twenty-eight patients (ATH group) with similar conditions underwent abdominal total hysterectomy (ATH) . The levels of IL-2 and IL-10 in serum of the two groups were measured at 1 day before operation and at 1 and 4 days after operation. Results ① The levels of IL-2 in the peripheral blood of the two groups were significantly lower than those of the preoperative one (P <0.05), but the levels of ATH were significantly lower than those of the LTH group (P <0.05) (P <0.05). The levels of IL-2 in the LTH group rose to the preoperative levels 4 days after operation (P> 0.05), and were significantly higher than those in the ATH group Compared with the difference was statistically significant (P <0.05). (2) The level of IL-10 in the peripheral blood of theATH group was significantly higher than that of the preoperative one (P <0.05), and decreased on the 4th day after operation, but still significantly higher than that before operation (P <0.05) The level of IL-10 in peripheral blood of 4 days was higher than that before operation, but the difference was not statistically significant (P> 0.05). Compared with ATH group, the difference was statistically significant (P <0.05). Conclusion: Compared with ATH, LTH has less trauma and little effect on immune function.
