目的观察特发性中枢性性早熟(ICPP)女孩瘦素、脂联素与胰岛素抵抗的关系。方法以2004-06—2006-08在临海市第一人民医院就诊的ICPP女孩50例为研究对象,以同期在该院儿保科接受体检的与之年龄相匹配的34名健康女孩作为正常对照组。均采取空腹静脉血测定血糖,同时空腹抽取静脉血用放免法测试脂联素、瘦素、血浆胰岛素(FINS)、促卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)以及三酰甘油[甘油三酯(TG)]、高密度脂蛋白胆固醇(HDL-C)。结果ICPP女孩FINS、胰岛素抵抗指数(HOMA-IR)、瘦素明显高于正常对照组(P<0.05),E2、脂联素明显低于正常对照组(P<0.05)。两两相关分析显示:ICPP组体重指数与HO-MA-IR呈显著正相关(r=0.37,P<0.05),与脂联素呈显著负相关(r=-0.45,P<0.01),与瘦素呈显著正相关(r=0.30,P<0.05),E2与HOMA-IR呈显著负相关(r=-0.36,P<0.05)。结论胰岛素抵抗(IR)是引起ICPP儿童脂联素水平下降、瘦素升高的原因,这为认识ICPP女孩的IR提供了新的思路。
Objective To investigate the relationship between leptin, adiponectin and insulin resistance in girls with idiopathic central precocious puberty (ICPP). Methods Fifty-three ICPP girls who were treated in Linhai First People’s Hospital from June 2004 to June 2006 were enrolled in this study. A total of 34 healthy girls whose physical examinations matched their age in the same period were selected as normal control group . Fasting venous blood was taken to measure blood glucose, while fasting venous blood was collected for determination of adiponectin, leptin, plasma insulin (FINS), follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol ) And triglyceride [triglyceride (TG)] and high-density lipoprotein cholesterol (HDL-C). Results The FINS, HOMA-IR and leptin in ICPP girls were significantly higher than those in normal controls (P <0.05). The levels of E2 and adiponectin in ICPP girls were significantly lower than those in normal controls (P <0.05). Correlation analysis showed that there was a significant positive correlation between body mass index and HO-MA-IR in ICPP group (r = 0.37, P <0.05) and negative correlation with adiponectin (r = -0.45, P <0.01) Leptin showed a significant positive correlation (r = 0.30, P <0.05). There was a significant negative correlation between E2 and HOMA-IR (r = -0.36, P <0.05). Conclusion Insulin resistance (IR) is the cause of the decrease of adiponectin and the increase of leptin in ICPP children, which provides a new idea for understanding the IR of ICPP girls.
