目的:为提高肾上腺肿瘤的超声诊断率,本文对肾上腺肿瘤的超声、CT或MR检查进行回顾性分析,并与病理对照。材料与方法:怀疑肾上腺肿瘤者35例,手术前均进行了超声、CT或MR检查,仪器使用ALOKA650,探头频率35MHz。结果:肾上腺肿瘤28例,超声发现27例,检出率为964%;另7例非肾上腺肿瘤超声误诊为肾上腺肿瘤。超声定位定性正确率为60%,CT或MR为572%;仅定位准确率超声为771%,CT或MR为829%。结论:分析各类肾上腺肿瘤的声像图特征,发现嗜铬细胞瘤、肾上腺皮质腺瘤、髓样脂肪瘤、肾上腺囊肿图像特异性强,易诊断;其它类型肾上腺肿瘤缺乏典型图像,需结合病史或其他检查综合判断。
OBJECTIVE: To improve the diagnostic rate of adrenal tumors, we performed a retrospective analysis of the adrenal tumors with ultrasound, CT, or MR and compared them with pathology. Materials and Methods: Thirty-five suspected adrenal tumors were examined by ultrasound, CT, or MR before surgery. The instrument used ALOKA650 and the probe frequency was 35MHz. Results: There were 28 cases of adrenal tumors, 27 cases were found by ultrasound, and the detection rate was 96.4%. The other 7 cases of non-adrenal tumors were misdiagnosed as adrenal tumors. The accuracy of ultrasound positioning was 60%, and the CT or MR was 57. 2%. The accuracy of ultrasound was only 77.1% for positioning accuracy, and 82.9% for CT or MR. Conclusion: The sonographic features of various types of adrenal tumors were analyzed. It was found that pheochromocytoma, adrenocortical adenoma, myeloid lipoma and adrenal cyst were highly specific and easily diagnosed; other types of adrenal tumors lacked typical images and required a combination of medical history.
