患者女性,47岁,因发现高血压1年而入院。曾用噻嗪类利尿剂、甲基多巴等连续治疗8月无效。停药后发生多汗、皮肤潮红、神经质、心悸、多尿、面肿、面部多毛、腹围增加。入院时血压220/120mmHg,肥胖、满月面、锁骨上脂肪增加,鬍须明显,手震颤。无紫纹及皮肤变薄,新近作的子宫切除术切口瘢痕色素未增加。实验室检查:血糖15.8m mol/L(284mg/dL),血钾2.6mEq/L,血浆皮质醇显著增高为1.31～1.55
The female patient, 47 years old, was admitted to hospital for having been diagnosed with hypertension for 1 year. Consecutive treatment with thiazide diuretics and methyldopa was ineffective for 8 months. After treatment, sweating, skin flushing, nervousness, palpitations, polyuria, bloated face, facial hairiness, and abdominal circumference increased. Blood pressure 220/120mmHg on admission, obesity, full moon, increased clavicle fat, obvious beard, hand tremors. No purple striae and thinning of the skin, recent scarring of the hysterectomy incision did not increase. Laboratory examination: blood glucose 15.8m mol/L (284mg/dL), serum potassium 2.6mEq/L, and plasma cortisol significantly increased by 1.31 to 1.55
