目的分析上海市嘉定区胃癌发病的危险因素,为该区胃癌的干预提供科学依据。方法选取2010年5月至2011年8月上海市嘉定区诊断新发胃癌病例224例。进行1:1全人群配对病例对照研究。结果单因素分析结果显示,重盐饮食,腌制食品,常吃宵夜,调料糖、料酒、味精使用较多,吸烟,饮酒,经常失眠,体力劳动强度过大,胃部疾病家族史,肿瘤家族史可增加患胃癌的风险;而多吃海产品,调料醋、辣椒、芡粉使用较多,新鲜蔬菜,豆制品,饮茶,经常散步,性格开朗则是胃癌的保护性因素。多因素分析结果发现,吸烟、常吃腌制食品、胃息肉患病史和家族肿瘤史是影响该区居民胃癌发生的主要危险因素,而常饮茶、多吃新鲜蔬菜、多吃豆类食品、性格开朗对于胃癌的发生有保护性作用。结论不当的饮食习惯和生活方式是该区居民胃癌高发的危险因素,积极在全人群开展健康教育,普及防癌抗癌知识,倡导健康的饮食习惯和生活方式对控制该区胃癌发生发展有重要的意义。
Objective To analyze the risk factors of gastric cancer in Jiading District of Shanghai and provide a scientific basis for the intervention of gastric cancer in this area. Methods From May 2010 to August 2011, 224 cases of newly diagnosed gastric cancer were diagnosed in Jiading District of Shanghai. A 1: 1 whole population matched case-control study. Results The results of univariate analysis showed that heavy salt diet, preserved food, evening appetizer, seasoning sugar, cooking wine and MSG were more used, smoking, drinking, frequent insomnia, excessive manual labor, family history of gastric diseases, Family history can increase the risk of gastric cancer; and eat more seafood, spices vinegar, peppers, Fen powder use more fresh vegetables, soy products, tea, often walking, cheerful is a protective factor for gastric cancer. Multivariate analysis found that smoking, eating pickled foods, history of gastric polyps and family history of cancer is the main risk factors affecting the incidence of gastric cancer in residents of this area, and often drink tea, eat more fresh vegetables, eat more legumes , Cheerful for the occurrence of gastric cancer have a protective effect. Conclusions Improper eating habits and lifestyle are the risk factors for high incidence of gastric cancer in residents of this district.
