目的研究稽留流产临床发病危险因素以及干预对策。方法选取130例稽留流产孕妇作为研究组。正常妊娠孕妇128例作为对照组,采取回顾分析法,收集两组发病危险因素的资料,包括遗传因素、感染因素、性激素因素以及免疫抗体因素;检测孕妇激素水平,包括FSH、T、PRL、P、LH等;检测孕妇的ACA、AEIVIA、ASA、AOA、Ah CGA、Ah PA等指数,检测孕妇体内感染情况;检测胚胎绒毛,研究相关染色体影响因素。结果研究组孕妇的T、E_2水平高于对照组孕妇,PRL、P水平低于对照组(P<0.05)。研究组孕妇的AEIVIA、Ah CGA、AOA和ACA指数高于对照组(P<0.05)。研究组孕妇染色体异常35例,占26.92%。对照组孕妇染色体异常18例,占14.06%,两组染色体异常率有统计学差异(χ~2=6.5353,P=0.0106)。研究组孕妇TORCH、UU、CT感染率明显多于对照组(P<0.05)。结论孕前检查母体感染情况和染色体异常,孕期维持母体激素水平,加强免疫力,可以有效降低孕妇稽留流产的发生率。
Objective To study the clinical risk factors of missed abortion and intervention measures. Methods 130 cases of missed abortion pregnant women as the research group. 128 pregnant women with normal pregnancy as control group, retrospective analysis was used to collect information on risk factors of two groups, including genetic factors, infection factors, sex hormone factors and immune antibody factors; test hormone levels of pregnant women, including FSH, T, PRL, P , LH, etc .; detection of pregnant women, ACA, AEIVIA, ASA, AOA, Ah CGA, Ah PA and other indicators to detect pregnant women in vivo infection; detection of embryo villi, chromosome related factors. Results The levels of T and E_2 in pregnant women in study group were higher than those in pregnant women in control group, and the levels of PRL and P were lower than those in control group (P <0.05). The AEIVIA, Ah CGA, AOA and ACA index of pregnant women in the study group were higher than those in the control group (P <0.05). The study group had 35 cases of chromosomal abnormalities in pregnant women, accounting for 26.92%. There were 18 cases of chromosomal abnormalities in pregnant women in the control group, accounting for 14.06%. There was a significant difference in chromosomal abnormalities between the two groups (χ ~ 2 = 6.5353, P = 0.0106). The infection rate of TORCH, UU and CT in study group was significantly higher than that in control group (P <0.05). Conclusion Pre-pregnancy check maternal infection and chromosomal abnormalities during pregnancy to maintain the level of maternal hormones and enhance immunity, can effectively reduce the incidence of missed abortion in pregnant women.
