目的:探讨miR-146a在慢性免疫性血小板减少症(ITP)患者外周血单个核细胞(PBMC)的表达及其临床意义。方法:选取本院血液科收治的ITP患者35例,选取同期健康者38例作为对照组,比较2组研究对象miR-146a在PBMC和血清中表达量的差异;比较2组血小板计数(PLT)、细胞因子、T细胞亚群的差异;并对ITP患者PBMC中miR-146a的表达量与血清中miR-146a的表达量、PLT、细胞因子、T细胞亚群进行相关性分析。结果:ITP组PBMC中miR-146a的表达量和血清中miR-146a的表达量均显著低于对照组[(0.071±0.004)%∶(0.089±0.008)%,(1.549±0.412)%∶(4.485±0.396)%,均P<0.01]。ITP组外周血中的PLT和Treg显著低于对照组,而IFN-γ、TNF-α、IL-2、Th22显著高于对照组(均P<0.05)。ITP患者PBMC中miR-146a的表达量与血清中miR-146a的表达量、PLT、Treg呈正相关,与IFN-γ、TNF-α、IL-2呈负相关(均P<0.05)。结论:ITP患者PBMC中miR-146a的表达量降低,IFN-γ、TNF-α、IL-2、Th22升高。ITP患者PBMC中miR-146a表达与血清中miR-146a的表达量、PLT、Treg呈正相关,与IFN-γ、TNF-α、IL-2呈负相关。
Objective: To investigate the expression of miR-146a in peripheral blood mononuclear cells (PBMCs) in patients with chronic immunological thrombocytopenia (ITP) and its clinical significance. Methods: Thirty-five patients with ITP admitted to our department of hematology were enrolled. Thirty-eight healthy subjects were selected as the control group. The expression of miR-146a in PBMC and serum was compared between the two groups. The platelet count (PLT) , Cytokines and T cell subsets. The correlation between the expression of miR-146a and the level of miR-146a, PLT, cytokines and T lymphocyte subsets in PBMC of patients with ITP was analyzed. Results: The expression level of miR-146a and the level of miR-146a in PBMC of ITP group were significantly lower than that of the control group [(0.071 ± 0.004)%: (0.089 ± 0.008)%, (1.549 ± 0.412)% 4.485 ± 0.396)%, all P <0.01]. The levels of PLT and Treg in peripheral blood of ITP group were significantly lower than those in control group, while the levels of IFN-γ, TNF-α, IL-2 and Th22 in ITP group were significantly higher than those in control group (all P <0.05). The expression level of miR-146a in PBMC of ITP patients was positively correlated with the expression of miR-146a, PLT and Treg in serum, and negatively correlated with IFN-γ, TNF-α and IL-2 (all P <0.05). Conclusion: The expression of miR-146a in PBMC of patients with ITP is decreased and the levels of IFN-γ, TNF-α, IL-2 and Th22 are increased. The expression of miR-146a in PBMC from patients with ITP was positively correlated with the expression of miR-146a, PLT, Treg and negatively correlated with IFN-γ, TNF-α and IL-2.
