目的了解社区肿瘤患者中抑郁的患病状况,探讨肿瘤患者发生抑郁的可能影响因素。方法应用抑郁自评量表Zung对184名社区肿瘤患者及家属进行调查。结果社区肿瘤患者中抑郁症患病率28.3%,家属为15.8%。影响肿瘤患者抑郁评分和抑郁患病率的主要因素为诊断时肿瘤分期、KPS评分、疼痛、肿瘤有否转移、复发、社会支持及家属抑郁评分和抑郁患病率(P<0.05),与患者抑郁程度相关因素主要为社会支持利用度和家属的抑郁程度(P<0.05)。社区肿瘤患者的ZSDS评分与主观支持、对社会支持利用度、社会支持总分呈负相关(r<0,P<0.01);抑郁程度与社会支持利用度、社会支持总分呈负相关(r<0,P<0.05)。社区肿瘤患者与家属两者的ZSDS评分、抑郁的有无、抑郁程度均呈正相关(r>0,P<0.01)。结论肿瘤患者抑郁状况与疾病状况、社会支持、家属抑郁状况密切相关,对于那些中晚期、体能状况差(KPS评分低)、有疼痛、有转移、复发,缺乏社会支持的患者应加强对心理健康指导和支持。并将对社区肿瘤患者及其家属的心理干预作为一个整体。
Objective To understand the prevalence of depression in community cancer patients and explore the possible influencing factors of depression in cancer patients. Methods A total of 184 community cancer patients and their relatives were investigated by Zung depression self-rating scale. Results The prevalence of depression in community cancer patients was 28.3% and that of relatives was 15.8%. The main factors influencing the depression score and the prevalence of depression in cancer patients were the tumor staging, KPS score, pain, tumor metastasis, recurrence, social support, family member depression score and prevalence of depression (P <0.05) The main factors related to the degree of depression were the degree of social support and family members’ depression (P <0.05). ZSDS scores of community cancer patients were negatively correlated with subjective support, utilization of social support, and total score of social support (r <0, P <0.01); depression was negatively correlated with social support utilization and social support score <0, P <0.05). There was a positive correlation between ZSDS score, presence or absence of depression and depression among community cancer patients and their relatives (r> 0, P <0.01). Conclusion The depression status of cancer patients is closely related to disease status, social support and depression status of family members. Patients with poor physical fitness (low KPS), pain, metastasis, recurrence and lack of social support should strengthen mental health Guidance and support. And will community cancer patients and their families psychological intervention as a whole.
